4. Regarding angina and myocardial infarction:
For treatment under LA, Pre operative glyceryl trinitrate should be considered in patients with angina .
LA solutions with epinephrine may be safely used with aspirating syringres to avoid intra vascular injections.
The death rate of myocardial infarction associated with GA is 50%.
GA is particularly dangerous for patients who have had an infarction in the previous 6 months.

3. Arrythmias:

. They should be controlled before GA / Sedation ( Ca- channel blockers, beta blockers or cardiac glycosides are used for management along with anti coagulants).

. Use additional supplemental oxygen while using conscious sedation.

. LA with adrenaline should be avoided.

2. Regarding Hypertension:

For treatment under local anesthesia, solutions with epinephrine may be used safely with aspirating syringes to reduce the risk of intravascular injection ( which may cause hypertension, arrhythmia or trigger angina in susceptible patients).

BP should pe controlled before treatment under GA/Sedation.
If the diastolic pressure is persistently above 110 mmHg, the risk of stoke and MI is increased.

Dental relevance of the CVS conditions:

1. Regarding congenital and rheumatic heart disease, it is sensible to restrict antibiotic prophylaxis to patients, undergoing sub gingival periodontal therapy or surgical procedures, in whom the risk of developing Endocarditis is the highest. Those include patients who have/had:

A. Endocarditis.
B. Cardiac valve replacement surgery
C. Surgically constructed systemic/pulmonary shunt or conduit.

MANAGEMENT:

A. Good oral hygiene and regular dental care For disease prevention.
B. Amoxicillin 3g orally 1 hour before the procedure OR Clindamycin 600mg orally (if allergic to peniciilins).
