Smile Makeover 😃 | Conservative Anterior Rehabilitation
The patient presented with an aesthetic concern regarding the anterior teeth, particularly the space between the central incisors, a carious left lateral incisor, a fractured incisal edge of the right lateral incisor, and overall tooth discoloration/deposits.
Clinical Findings & Diagnosis
Mild midline diastema between the central incisors.
12: Uncomplicated crown fracture involving the incisal edge.
22: Carious lesion with a diagnosis of reversible pulpitis, along with Grade I mobility, associated with bruxism.
Generalized deposits requiring scaling and polishing.
Treatment Plan:
After discussing the available options with the patient, a conservative and relatively quick approach was selected:
1. Diastema closure :
The space was closed using direct composite. Orthodontic treatment and veneers were also discussed as alternative options but the patient wanted a quick & conservative approach.
2. 12 – Incisal edge build-up The fractured incisal edge of 12 was restored using a direct composite build-up, followed by finishing and polishing to achieve a natural appearance.
3. 22 – Endodontic treatment & crown Although the tooth initially presented with reversible pulpitis, an elective RCT was carried out, followed by composite build-up and subsequent crown rehabilitation.
4. Management of mobility : Considering the Grade I mobility of 22 and the patient's bruxism, the tooth was splinted for one week before definitive crown placement. At follow-up, a noticeable reduction in mobility was observed, after which the final crown was placed.
5. Bruxism management As parafunctional forces could compromise the restorations and increase stress on the teeth, a night guard was fabricated and the patient was instructed to wear it at night.
Final Outcome:
The overall treatment focused on achieving an improved anterior smile through a conservative, minimally invasive approach.
Treatment performed: Scaling & Polishing → Diastema Closure → 12 Incisal Edge Composite Build-up → RCT & Rehabilitation of 22 → Splinting → Crown Placement → Night Guard
