HOPI -
14 years old young boy with complaint of discolouration of front teeth and food impaction in upper posterior teeth
O/E • White patches , opacities on upper anterior teeth
• Upper 1st molar was carious ( class2)
•Palpation - negative
• TTP - negative
• Mobility - N/A
• Affected teeth - Vital ( clinically and radiographically)
Dx - CHRONOLOGICAL ENAMEL DEFECT
Tx - Restorations of anterior teeth with composite or veneers
Restorations of upper 1st molar ( bilaterally ) with composite or GIC ( Done )
•DESCRIPTION OF CASE
⏱️ Time-linked
Occurs during specific stages of enamel formation (secretory or maturation stage).
🦷 Symmetrical Pattern
Often affects teeth forming at the same time (e.g., all first molars + incisors = “6-year molars”).
⚠️ Causes
Fever, infections, malnutrition, trauma, systemic illness, medications (e.g., tetracycline), or prematurity.
🔍 Appearance
Horizontal lines, pits, grooves, opacities, or complete enamel loss.
📍 Location on Tooth
The position of the defect on the crown tells you when the insult occurred (based on tooth development charts).
