When plaque is not adequately removed through proper oral hygiene practices such as brushing and flossing, it can mineralize and harden over time, forming calculus.

Factors such as poor oral hygiene, inadequate brushing and flossing, infrequent dental cleanings, smoking, and certain medical conditions or medications can increase the likelihood of calculus formation.

Regular professional dental cleanings are necessary to remove these hardened deposits, as they cannot be eliminated through regular oral hygiene practices alone.
