Why Can’t Teeth Heal Themselves?

The key reason lies in tooth structure and biology:

1. Lack of Living Cells in Enamel
The outermost layer of the tooth, enamel, is the hardest and most mineralized tissue in the body. However, it is completely acellular—it contains no living cells or blood vessels. This means it cannot regenerate, repair, or remodel like bone or skin.

2. No Blood Supply or Nerve Supply in Enamel
Enamel doesn’t receive a blood supply, which is essential for delivering immune cells and healing factors. Once enamel is eroded or decayed, the body has no natural mechanism to restore it.

3. Dentin Has Limited Regeneration
The layer beneath enamel, dentin, is slightly more biologically active. Odontoblasts (specialized cells within the pulp) can produce reparative or tertiary dentin in response to mild stimuli like trauma or caries. However, this is limited and slow, and cannot reverse significant damage.

4. Tooth Pulp Is Delicate
If decay or trauma reaches the pulp, inflammation or infection can occur, leading to pulp necrosis. Unlike other tissues, once pulp dies, the tooth loses its vitality and requires endodontic treatment (root canal).

Ito ay mga central incisors hindi ng 20 years old or 30 or 50 or even 60 kundi ng isang 11 year old child, and yes permanent teeth po ito. Yes, wala nang papalit dito. ‘Edi doc bungi na po sya?’ Yes bungi na sya sa front. And yes, magpupustiso na sya dahil hindi pa po pwede i fixed bridge ang ganitong edad. What is alarming here is that at a very young age, the patient already needs to wear dentures to restore the missing teeth. 🥹

Actually sa practice ko, napapansin ko na pabata ng pabata ang mga pasyente ko who need extraction of their permanent teeth, not because ito na lang ang solusyon ha? We can always do pulp therapy. But because this is the only solution na pasok sa budget ng ibang patients. And also they are not well informed about treatments like root canal, pulpotomy and even pulpectomy. 🦷

What can we do to avoid this situation? Early detection and prevention. By what? Brushing, flossing, regular cleaning, and dental check up. Establishing a routine sa bahay is also very important, that’s why you should start them young! Alongside the Oral Hygiene routine, it is essential to visit your dentist to prevent gum disease and cavities from forming. Itong 2 ito (cavity & gum disease) ang mga rason kung bakit maaga at mabilis nasisira ang ngipin natin. 🪥

So when is the first visit ba dapat ng anak ninyo sa dentist? It is when the first tooth erupts. Para di confusing, kahit atleast 1 year old si baby. We can do brushing, fluoride application, and dental check up!
