Orthodontic Transfer Case | 18-Month Outcome
Sharing a challenging transfer case that required comprehensive biomechanical correction and interdisciplinary planning.
The patient initially presented with ongoing orthodontic treatment but with unresolved concerns: missing 44, 11 and 12 (previously extracted, likely due to a mesiodens), and a labially impacted 13. The existing setup did not adequately address space management, midline discrepancy, or eruption guidance.
Treatment approach:
- Extraction of 24 to facilitate space redistribution
- Controlled lateral movement of 21–23 to the left for midline correction
- Surgical exposure and orthodontic traction of impacted 13
- Recontouring of 13 to simulate a lateral incisor morphology
- Pontic substitution for 11 to restore esthetics and function
Key considerations: Achieving symmetry in the anterior segment required careful torque control, space closure mechanics, and esthetic finishing, especially with canine substitution and pontic integration. Anchorage management and force vector control were critical throughout traction and alignment phases.
Outcome: In just 18 months, we achieved functional occlusion, improved upper dental midline alignment, and a significantly enhanced smile esthetic despite the initial limitations of the case.
Transfer cases can be complex, but with proper diagnosis, biomechanics, and clear treatment objectives, predictable outcomes are still achievable.
To God be the glory!
