TMJ Pain Deserves More Than a Guess. 🦷

Jaw pain, painful click, limited mouth opening, facial pain, and ear pressure are not symptoms to dismiss — they are signals worth investigating with precision.

MRI of the temporomandibular joint provides detailed evaluation of disc position, condyle morphology, joint effusion, and early degenerative changes that clinical examination alone cannot reveal.

TMJ MRI may be indicated when patients present with:

• Persistent jaw or facial pain
• Audible painful clicking and popping
• Restricted or painful mouth opening
• Recurrent headaches of orofacial origin
• Ear symptoms or a sensation of pressure
• Locking episodes

Pain Is Not Always a Reflection of Structural Damage.

A patient may present with a completely normal MRI or CBCT scan and still experience severe, life-altering pain. Another may show significant joint degeneration on imaging yet report little to no discomfort. This disconnect is not unusual — it is one of the most important principles in Orofacial Pain and TMD, pain does not equal structure.

Pain is a complex biological process shaped by far more than anatomy. The nervous system, muscles, inflammation, sleep, psychological stress, and the brain’s interpretation of sensory input all play a role in how pain is experienced. The TMJ, muscles of mastication, and surrounding neural structures do not function as isolated parts — they operate as an interconnected system, and disruption anywhere within that system can produce or amplify pain regardless of what an image shows.

This is why imaging alone is never sufficient for diagnosis. Effective Orofacial Pain management requires understanding the full picture — not just what is visible on a scan, but the entire biological and neurological context behind a patient’s pain experience.

The biggest mistake in diagnosing jaw pain? Assuming every “toothache” comes from a tooth.
Many patients spend years — and hundred thousand pesos — on root canals, crowns, or extractions for pain that never fully resolves. Not because the treatment was poorly done, but because the tooth was never the true source of the problem.

Pain in the teeth, face, temple, ear, or neck can often be referred — originating from overworked or inflamed jaw muscles, a stressed temporomandibular joint, or surrounding structures. Clenching, grinding, bite imbalance, and chronic stress can all cause the muscles of mastication to “throw” pain signals far from their actual origin. The tooth gets treated. The pain returns.
This is why accurate diagnosis is everything.

Orofacial pain rarely has a single, obvious cause. A cracked tooth, muscle dysfunction, joint inflammation, nerve irritation, sleep-related bruxism, airway compromise, and stress-driven tension can all present with nearly identical symptoms. Treating one without evaluating the others is where care so often falls short.
