🚨 TOOTH PAIN THAT KEEPS PATIENTS AWAKE AT NIGHT? THINK AGAIN.

A patient walks in with:

Severe throbbing pain

Tooth feels “high” on biting

Extreme tenderness to percussion

Most students jump to periodontal causes…

❌ But here’s the catch — the tooth is NON-VITAL

👉 This is a classic Periapical Abscess

🧠 HIGH-YIELD SNAPSHOT (INBDE GOLD)

🔴 Etiology

Untreated caries / trauma → Pulp necrosis

⚫ Key Diagnostic Clue

Non-vital tooth (MOST IMPORTANT)

🟡 Clinical Features

Severe pain

Tender on percussion

Swelling ± sinus tract

Tooth feels “extruded”

🔵 Radiograph

Early → Normal

Late → Periapical radiolucency

🟣 Treatment

RCT / Extraction + Drainage

🚫 Antibiotics NOT routine

✅ Only if fever / cellulitis / systemic spread

⚠️ EXAM TRAP YOU MUST REMEMBER

👉 Periapical abscess = NON-VITAL tooth
👉 Periodontal abscess = VITAL tooth (usually)

💥 This single line can save you marks.

❓ MCQ

A 32-year-old patient presents with severe pain in a mandibular molar. The tooth is highly tender to percussion and feels elevated during occlusion. Clinical examination shows localized swelling near the apex. Pulp vitality test reveals no response.

What is the most appropriate management?

A. Scaling and root planing
B. Antibiotic therapy alone
C. Root canal treatment with drainage
D. Occlusal adjustment only

If your tooth pain feels worse at night, you’re not imagining it—there’s a real reason behind it.

When you lie down, blood flow to the head increases, raising pressure inside an already inflamed tooth. Because the dental pulp is enclosed in a small, rigid space, even a slight rise in pressure can intensify pain signals.

At the same time, pain becomes more noticeable at night. With fewer distractions and reduced sensory input, the brain focuses more on discomfort—making the toothache feel sharper and more persistent.

There’s also a circadian effect. Pain sensitivity tends to increase at night, and as the effects of pain relief wear off, underlying issues like pulp inflammation, deep decay, or infection become more evident.

This is more than just discomfort. Persistent or night-time tooth pain is often a sign of an underlying dental problem that needs timely evaluation.

Palinis na po

“A rotten tooth 🦷 … almost destroyed her 🫀💀heart.”

A 47-year-old woman had:

🤒 Low-grade fever for weeks
🤢 Fatigue, loss of appetite
🍓 Strange red spots on her palms

She thought it might be an STI (or STD / sexual transmitted diseases). It wasn’t.

It was her heart. 🫀

🦷🦷🦷🦷🦷🦷🦷🦷🦷🦷🦷🦷

What actually happened?

She had severe untreated tooth decay for years:

* Pain on and off
* Took painkillers, never fixed the root cause
* Poor oral hygiene (late-night sugar + no brushing)

Bacteria 🦠 from her mouth kept entering her bloodstream. 🩸

Usually, the body can handle this.

But over time…

👉 The bacteria stuck to her heart valve 📌
👉 Formed an infected mass (called vegetation)
👉 Started damaging the valve

This condition is called

🫀 Infective Endocarditis (IE)

🦷🦷🦷🦷🦷🦷🦷🦷🦷🦷🦷🦷

Clues doctors found

* Chronic fever
* Bad oral condition (decay, gum infection)
🫀 Heart murmur (abnormal valve sound)
* Painful red nodules on fingers (Osler nodes)
* Flat red spots on palms (Janeway lesions)🍓
* Echocardiogram showed infection on the valve

🦷🦷🦷🦷🦷🦷🦷🦷🦷🦷🦷🦷

Why it got worse

Her blood sugar 🍭was 320 mg/dL → likely undiagnosed diabetes

High sugar = weaker immune defense
→ easier for bacteria to spread and grow🚨

🦷🦷🦷🦷🦷🦷🦷🦷🦷🦷🦷🦷

Why this matters

Your mouth is not separate from your body.📌🫶🏻🫀

It’s a direct gateway to your bloodstream.🩸

Untreated dental infections can spread to:

* ❤️ Heart (endocarditis)
* 🧠 Brain (abscess, stroke)
* 🫁 Lungs

🦷🦷🦷🦷🦷🦷🦷🦷🦷🦷🦷🦷🦷

Simple prevention📌

* Brush 2× daily
* Floss (this removes what brushing can’t)
* Fix cavities early
* Don’t ignore bleeding or swollen gums
* Avoid sugar + sleep without brushing

🦷🦷🦷🦷🦷🦷🦷🦷🦷🦷🦷🦷🦷

🪥Tooth decay is not “just a tooth problem.”

In the wrong conditions, it can become a life-threatening heart infection. 💀🫀
