Overlays vs Crowns?

Why in our clinic we always opt to finish our root canaled teeth ( prone to fractures/cracking ) with Overlays instead of Crowns?

1. No need for big reduction of sound tooth structure.

2. We keep the enamel (the strongest layer of the tooth) at the cervical area which absorbs the flexural stress of chewing forces.

3. Easier to prepare, to take impression and to do proper isolation during cementation of Overlays

4. Removing all fractured points on original tooth and replacing it with Overlays helps reinforce the tooth, distributing chewing forces evenly, preventing further cracking and breaking .

Clinical judgment remains just as critical as x-ray findings when determining whether a tooth is saveable or requires more invasive treatment.

A patient presented with pain triggered by stimuli, which was relieved by medication. Upon examination, the tooth responded positive to cold test, with sensitivity resolving completely in under 10 seconds. These findings suggested a reversible pulpal condition, indicating that the tooth had the potential to recover with proper intervention.

Based on this, a BIOMIMETIC restoration was performed following gold-standard adhesive and restorative protocols.

The outcome was favorable: the patient experienced immediate relief after the procedure, with complete resolution of symptoms by the following day. Through careful diagnosis and conservative treatment planning, root canal therapy was avoided, preserving the vitality of the tooth.

A proper coronal seal / PRECISE FIT of final restoration is crucial because bacterial leakage from the oral cavity can re-infect the canal system even after a well-done root canal. Studies show that both a good root canal filling and a high-quality coronal restoration are essential for long-term endodontic success

This case presented with a previously placed crown and an infected root canal. A retreatment was performed for both the root canal and the crown.

The tooth was re-prepared to improve the margins, allowing the ceramist to fabricate a crown that merges smoothly with the tooth structure. Proper marginal adaptation is critical to ensure an adequate seal.

The final crown must not present overhangs or open margins, as these defects can allow bacterial leakage and compromise the coronal seal. Achieving a PRECISE FIT helps protect the root canal treatment and contributes to the long-term success of the tooth.

Saved Tooth, One Day at a Time

When people ask why we choose to save teeth in difficult and complicated cases, the answer is simple — because we have seen success.

When a procedure is done properly, a natural tooth can last for many years without major problems. Of course, there are exceptions. Sometimes microcracks cannot be detected on a 2D X-ray, especially those hidden beneath the roots. But despite these uncertainties, we choose to try.

In the end, we want to be able to answer our own “what if” questions.

What if it can survive?
What if it can be successful?

Rather than immediately concluding that extraction followed by a denture or implant is the only solution, we give the natural tooth a chance.

Because the more we see artificial replacements being placed, the more we are reminded of the value of preserving what is natural.

Saving a tooth isn’t just about treatment.
It’s about possibility, patience, and believing in preservation.

A non-extraction case involving a teen male who presented with early signs of tooth wear. Treatment included alignment with self-ligating braces and restoration of worn canine and anterior teeth to re-establish proper guidance.

Proper alignment of the teeth is important not only for aesthetics but also for healthy function. Aligned teeth act like an “anti-aging” factor for the mouth, helping them wear down more slowly compared to mal aligned teeth. When the jaw is properly guided during function and the bite is balanced, chewing forces are distributed evenly. This allows the teeth to age more slowly, keeps the muscles more balanced, and helps prevent TMJ dysfunction.

Here's another instance of TOOTH WEAR where the patient predominantly expresses concerns about aesthetics, with functionality being a secondary issue. We began the treatment planning process with a Motivational Mock-Up to provide the patient with a visual representation of the anticipated final results. The treatment planning commenced with a MOTIVATIONAL Mock Up, during which no preparation or crown replacement was performed. This step serves solely to confirm the shade, shape, and occlusal balance(bite), providing a preliminary representation of the anticipated final appearance and bite for the patient.

The goal of this treatment is to RESTORE the tooth surface that has been lost due to aging and wear.
