First dental visit would often constitute oral examination. After mouth examination and assessment, we discuss treatment needs and options to the parents. In treatment planning, we first consider the ideal. Ideal treatment would include all measures to save the baby teeth. Alternative options are also discussed and certain modifications can be made depending on parents' preference, and other factors.
Baby teeth are important for the child's proper growth and development, nutrition, esthetics and self-esteem, and it serves as the guide for the permanent. On instances where early extraction of a baby tooth is necessary, a space maintainer can be placed to minimize drifting of the adjacent teeth. However, patients attitude and cooperation is also a consideration.
This is a case of a 4-year- old patient diagnosed with Global Developmental Delay. Chief complaint was pain and swelling on the lower molar areas (left and right). Radiographic examination revealed, lower right second molar was beyond repair, extraction was indicated. Space maintenance was offered, however, the parents thought child might not be able to tolerate a foreign material inside the mouth. Hence, none was placed. They were advised close monitoring and periodic visits to help child be more familiar and comfortable in dental visits and procedures.
Procedures performed were:
1. Intraoral Pre- and post-operative photos
2. Pulpotomy and Stainless steel crowns on tooth nos 74, 85
3. Preventive Resin Restoration on tooth nos 55, 65
4. Pit and sealant on tooth nos 54, 64
5. Pulpectomy and stainless steel crown on tooth 75
6. Extraction of tooth 85
7. Oral propfissurehylaxis and fluoride varnish application
Patient was recalled 2 weeks after the procedure. Extraction site was healing well. No signs of inflammation or erythema inside the oral cavity. Parents were informed periodic visits to be every 3 months.
