Learning never stops!

I always want to be updated and abreast with new knowledge in order to serve my patients better. Last week I, and some other colleagues from our country, were privileged to be part of the pioneer batch of the Pediatric Airway and Myofunctional Orthodontics Mini Residency organized by ! We have learned so much from Dr. German and Ms. Fabiola. I hope we will be able to apply these new learnings to our patients. 5 out of 29 participants are Filipinos.

Treatment options vary from patient to patient. During initial consultations; we do history taking and take diagnostic exams like xrays to be able to assess the case better. After initial assessment, ideal treatment plan and other probable options are discussed.

These are 2 cases where the chief complaints are (almost) the same. Tooth 75 (lower left primary second molar) presented with large decay and radiolucency in the xray (indicative of infection).

2 options were presented:

1. Pulpectomy (complete removal of the pulp/nerve) and Stainless Steel Crown
2. Extraction ans placement of a suitable space maintainer

Consultation visits are important for proper discussion of treatment needs and options. It can sometimes be overwhelming for parents but totally necessary. Treatment planning for oral rehabilitation is important, be it for conventional (multiple visits) or under sedation.

Maintenance after completion of rehabilitation is equally important to maintain good oral health and a caries-free oral environment.

Bring children to a pediatric dentist as soon as the first tooth comes out or by the time of their first birthday. Periodic visits should be at every 3 to 6 months, depending on caries susceptibility.

First dental visit would often constitute oral examination. After mouth examination and assessment, we discuss treatment needs and options to the parents. In treatment planning, we first consider the ideal. Ideal treatment would include all measures to save the baby teeth. Alternative options are also discussed and certain modifications can be made depending on parents' preference, and other factors.

Baby teeth are important for the child's proper growth and development, nutrition, esthetics and self-esteem, and it serves as the guide for the permanent. On instances where early extraction of a baby tooth is necessary, a space maintainer can be placed to minimize drifting of the adjacent teeth. However, patients attitude and cooperation is also a consideration.

This is a case of a 4-year- old patient diagnosed with Global Developmental Delay. Chief complaint was pain and swelling on the lower molar areas (left and right). Radiographic examination revealed, lower right second molar was beyond repair, extraction was indicated. Space maintenance was offered, however, the parents thought child might not be able to tolerate a foreign material inside the mouth. Hence, none was placed. They were advised close monitoring and periodic visits to help child be more familiar and comfortable in dental visits and procedures.

Procedures performed were:

1. Intraoral Pre- and post-operative photos
2. Pulpotomy and Stainless steel crowns on tooth nos 74, 85
3. Preventive Resin Restoration on tooth nos 55, 65
4. Pit and sealant on tooth nos 54, 64
5. Pulpectomy and stainless steel crown on tooth 75
6. Extraction of tooth 85
7. Oral propfissurehylaxis and fluoride varnish application

Patient was recalled 2 weeks after the procedure. Extraction site was healing well. No signs of inflammation or erythema inside the oral cavity. Parents were informed periodic visits to be every 3 months.

Dental phobia, or odontophobia, is a common fear that can prevent people from seeking necessary dental care. It can stem from various factors, such as past traumatic experiences, fear of pain, or even the sounds and smells of a dental office. Some people also feel a loss of control or anxiety about potential bad news regarding their oral health.

There are different levels of dental anxietyβ€”some people may feel mildly nervous before an appointment, while others experience extreme panic that prevents them from going to the dentist altogether. This can lead to serious oral health issues, such as cavities, infections, or gum disease, which then require more invasive treatment, reinforcing the fear.

These are cases of 2 kids with dental phobia which resulted to multiple problems in the oral cavity.Β

Case 1: Patient (10y/o) was referred by a GP in Batangas City. Chief complaint was pain on the lower 1st permanent molar. Extraction was attempted but management became challenging, hence the referral.

Case 2: (Patient from a different clinic) Patient (7y/o) came to the clinic because of toothache on the lower 1st permanent molars. Patient did not have prior dental visits. Mom claims that child still feeds from the bottle (7 to 14 bottles per day). Majority of the primary teeth were exfoliated spontaneously due to severe decay. 4 permanent first molars are badly decayed as well. Behaviorwise, patient was very anxious during the initial visit.

Early and pleasant dental visits are very important at an early age. This is the foundation of a lifelong good oral health and relationship with dental practitioners.

Dental visits should start as early as the first teeth come out or by the first birthday. When in doubt, seek consult from licensed practitioners and not rely solely on what is written on the internet.

"All I want for Christmas is my 2 front teeth!"

Human beings have two sets of teeth: baby teeth and the permanent teeth. The baby teeth starts erupting at age 6 months up to 3 years. While the permanent teeth starts erupting at 6 years to 12 years of age. (Age of eruption varies. Ages are just an average range.)

The first permanent teeth to come out are the lower central incisors at age 6. While the upper central incisors come out at age 7. Sometimes, newly erupted permanent teeth are still oddly positioned and would often have wide gaps in between. THESE ARE STILL NORMAL.

Likewise, permanent first molars would also start erupting. These are new molars that come out after the 2 baby molars. They do not replace any baby tooth.

Parents should assure kids that these changes are normal! Some kids would get scared or conscious about these changes.

Still, regular visits to the dentist is warranted for close monitoring of the developing dentition. Early signs of malocclusion may be addressed if caught early.

Merry Christmas! πŸŽ„πŸŽ…

What are Pits and Fissure Sealants?

Naturally, our teeth has a lot of anatomical variations. Pits and fissures, especially on the molars are naturally present. Each tooth may have different quality and depths of pits and fissures. These anatomical feature makes plaque and food particles very retentive. If not removed and cleaned properly, it can be the start of cavity formation.

Pit and fissure sealants are placed on deep pits and fissures of the teeth. This is usually indicated for newly erupted, sound (no cavity) teeth. This is specially recommended for patients who have high caries risk to prevent caries formation. Since they are placed on the biting surfaces of the teeth, it is prone to wear and tear. It needs to be checked periodically and replaced if necessary.
