***Silver Modified Atraumatic Restorative Technique (SMART)***
A technique to arrest decay and seal teeth using two products: Silver Diamine Fluoride (SDF) and Glass Ionomer Cement (GIC) restoration.

Due to the patient's anxiety and uncooperative behavior, traditional restorative treatment was not feasible without resorting to advanced, higher-risk behavior management (sedation/general anesthesia). SMART was utilized as an interim management strategy to arrest dental caries and "buy time." This approach successfully avoids the immediate need for conscious sedation or general anesthesia, allowing the decay to be managed while the patient matures and develops the coping skills necessary for definitive restorations.

***Stainless Steel Crown (SSC)***
This pre-formed crown is made of biocompatible stainless steel and provides full coverage and protection for the affected tooth until it naturally exfoliates (falls out).

We placed SSC on patient’s primary first molar due to an interproximal cavity (decay located on the side or in between the teeth), a standard filling is often not enough. Because cleaning a cavity on the side requires removing a significant amount of tooth structure, a regular filling can easily crack or fall out under the pressure of chewing. Therefore, placing a SSC is the most reliable solution to maintain the tooth's function, prevent further damage, and hold space for the permanent tooth to erupt correctly.

A cavity in a baby tooth is not always just a temporary problem.

Beneath every baby tooth, a permanent tooth is developing. When tooth decay progresses deep enough to reach the pulp and cause infection, that infection can spread into the surrounding tissues and affect the developing permanent tooth underneath.

Studies have linked severe infections in primary teeth with developmental disturbances in permanent successor teeth, including enamel defects, altered eruption patterns, delayed eruption, and changes in tooth position.

This is one reason dentists work hard to preserve and treat baby teeth whenever possible rather than simply waiting for them to fall out.

Baby teeth play several important roles:
• Guide permanent teeth into their proper positions
• Maintain space for the adult teeth
• Support normal chewing and speech development
• Help ensure healthy development of the permanent teeth growing below

Ignoring a cavity because “it’s only a baby tooth” can sometimes lead to problems that persist long after that baby tooth is gone.

Good oral hygiene, regular dental examinations, fluoride exposure, and timely treatment of cavities remain the best ways to protect both primary and permanent teeth.

Clinical patience in action. 📈 Over the past four months (May to August; 1 tooth per month), our team successfully managed and completed a treatment of Strip Off Crown(SOC) for a 2-year-old patient without the use of sedation. We made it! 👏🤗 Slowly but surely! This milestone was made possible through consistent behavior guidance and an incredible partnership with the patient's parents, who remained calm and supportive throughout the journey. Strip off crown (SOC) is a cosmetic and functional restoration option for damaged or decayed primary (baby) front teeth. The procedure involves using a clear, tooth-shaped acetate shell (the "strip") as a mold to shape and place a tooth-colored composite filling material. Once the composite hardens, the strip is removed (the “off), leaving behind a natural-looking, full-coverage restoration (the “crown). Strip off crowns are a good option for restoring the appearance and function of front teeth that are visible when a child smiles.

Advantages:
1. Provides a natural-looking, tooth-colored restoration, important for front teeth.
2. Protects the entire tooth from further decay.
3. Allows the child to bite and speak properly.
4. Generally requires less tooth reduction compared to stainless steel crowns.
5. Lasts until the primary tooth naturally exfoliates.
