When to wear Hawley’s retainers? Can you wear it before braces?

Retainers are worn after braces. Its function is to retain the position of your teeth, in proper alignment and occlusion (bite) after orthodontic treatment. It is not suggested to wear before braces, not even as a fashion retainer. If worn without your dentist’s approval, it could possibly do more harm than good to your oral cavity.

Is it really necessary to remove 3rd molars? This has been a topic amongst dentists. In our POV, these are the factors to consider to have your 3rd molar (upper or lower) removed:

1) Impacted - the tooth did not fully erupted to its normal level of occlusion

2) Not upright - the tooth may be erupted diagonally, sometimes against the adjacent tooth that can cause cavity due to the fact that most food debris get stuck in between plus the difficulty to clean properly because it’s located at the most posterior part of your oral cavity / mouth.

3) It’s extensively rotten - it’s difficult to restore, sometimes dentist cannot remove all caries thoroughly which will only cause bacteria build up once forced to restore. Basically, the caries that were left behind can still affect the remaining tooth structure even with composite restoration on top.

4) For orthodontic purposes

5) It has no opposing teeth - it will only become supra erupted that may cause cheek biting or irritability

6) Mobile - no bone support left to hold the tooth in the socket

Do you have 3rd molars? This tooth by the way is commonly known as wisdom tooth. Most people has 4 of these, 2 on the upper and 2 on the lower.

During our first assessment, patient initially wants to save the tooth by doing a root canal treatment. I told her that sometimes 2nd premolars may have 2 roots, hence 2 canals, that we can only determine during the procedure because a panoramic x-ray only shows a 2D view, not three dimensional.

However today, for some reason, she opted to have it removed. Lo and behold, the tooth has two roots!

Yes, we can still save the tooth structure with such extensive caries. But, it will be really expensive.
