These 2 teeth could still have been saved through root canal treatment.
However, due to financial constraints, the patient opted for extraction. This patient had the first extraction in her life, now she will be needing a denture to replace the missing teeth.

The impacted wisdom tooth that caused damage to the side of the 2nd molar was not removed so that it can still erupt and may still be used by the patient. Braces would be able to move this wisdom tooth to its proper position and replace the extracted 2nd molar. This can be another option instead of a denture.

Lesson from this case 🦷

• Never neglect small cavities. Early treatment is simpler, less invasive, and more affordable. Once cavities become bigger, treatment becomes more complex and costly.

• Replacing missing teeth is essential. After extraction, a denture (or other replacement) is necessary. Even if the pain is gone, teeth will start to shift—leading to bite problems and long-term issues if left unaddressed.

Amalgam fillings are known for their strength and longevity, but there are times when upgrading them is necessary—especially when there are signs of recurrent decay.

In this case, the dark areas seen were amalgam stains, not decay. Amalgam can discolor the natural tooth over time. To preserve as much tooth structure as possible, we avoided further drilling and restored the tooth with composite fillings instead.

A more conservative approach, a healthier tooth, and a more natural smile. 🦷
