But we didn’t want to guess. An X-ray helped us identify another tooth that also needed restoration, allowing us to create a more complete treatment plan.

🦷 One Treatment Plan Doesn’t Always Fit Every Patient

This patient was previously advised to have all four first premolars removed to help prevent wisdom tooth impaction. However, after reassessment, his wisdom teeth still required removal—the lower wisdom teeth were horizontally impacted, while the upper wisdom teeth were fully erupted but not properly aligned in the dental arch.

The good news? ✨ We were able to close all the spaces created by the previous extractions, achieve a well-balanced bite, and give him a functional set of teeth—without needing a denture. 🥰

An impacted or poorly positioned wisdom tooth can damage the 2nd molar beside it, leading to decay, infection, pain, or even loss of the neighboring tooth.

The good news? Wisdom teeth generally don’t need to be replaced after removal—especially when they are poorly aligned, not fully erupted, or causing problems. But if the wisdom tooth damages the 2nd molar and that tooth can no longer be saved, a replacement may eventually be needed.

This patient initially came to us because of a toothache, which led to root canal treatment of her premolar. Before doing the RCT, we carefully considered her orthodontic needs—because **that tooth still needed to be preserved.**

🦷 **Root Canal Treatment: It’s Not Just About Saving the Tooth**

This central incisor underwent **root canal treatment** after the tooth became necrotic—or, in simple terms, the nerve inside the tooth had died.

One challenge with root canal-treated teeth, especially front teeth, is **color matching**. A tooth that has become non-vital can naturally become darker or change in shade, making it difficult to perfectly match the surrounding teeth with a simple restoration.

This patient also has another tooth that needs RCT, but treatment has not yet been completed.

💡 **Why do we often recommend a crown after RCT?**
Aside from improving the appearance and helping the tooth blend better with the rest of the smile, a crown can provide additional protection to a tooth that has become more susceptible to fracture after root canal treatment.
