To set an appointment, may we have the following so we can schedule your properly:
1) Name of inquirer
2) name of patient
3) brief concern
4) patient age and gender
5) contact person and number
6) which branch do u prefer to be scheduled
7) available date do u wish to be treated
*** if HMO holder (INTELLICARE /AVEGA)
kindly give us the details of your card
- name of patient
- company name
- account number
please give us ample time to verify your account and dental coverage with your HMO facility.
*** for CAREHEALTH PLUS holder
kindly have your LOA with your agent
