“Cavities don’t hurt… until it’s too late.”

In their early stages, dental caries (cavities) silently erode the enamel—the outer protective layer of the tooth—without causing any pain. This is because enamel has no nerves. By the time a cavity reaches the dentin or the pulp, where nerves and blood vessels reside, symptoms like sensitivity and pain begin. At this point, the damage is often extensive and may require invasive treatment like root canal therapy.

Don’t wait for pain. Early detection through regular dental check-ups is the only way to stop cavities before they become serious.

Clinical perspective:

1. Enamel Is Acellular & Non-Vital

Enamel, the hardest substance in the human body, contains no living cells, no blood vessels, and no nerves.
Once it's damaged—through decay, trauma, or erosion—there is no intrinsic healing mechanism. Unlike bone or skin, enamel cannot regenerate or remodel.

2. Dentin & Pulp: Limited Healing Capacity

Dentin is formed by odontoblasts, which can slowly deposit secondary or tertiary dentin in response to mild irritation. However, this response is localized and limited in capacity.

Dental pulp, being vascular and innervated, can mount an inflammatory response. But once infection sets in, necrosis often follows, requiring endodontic intervention.

3. Does Any Healing Happen at All?

Yes, but only to a very limited extent:

Remineralization of early enamel lesions is possible with fluoride and calcium/phosphate-rich saliva.

Tertiary dentin may form beneath shallow caries or trauma.

But once cavitation occurs or structural integrity is lost, biological healing is no longer possible.

This practice helps prevent cavities, particularly in areas that are hard to clean, and significantly reduces the risk of gum disease, such as gingivitis and periodontitis. Regular flossing also contributes to fresher breath by eliminating trapped debris, and good oral health is linked to overall health, potentially lowering the risk of heart disease and other systemic issues. To floss effectively, cut about 18 inches of dental floss, wrap the ends around your middle fingers, and use your thumbs and index fingers to guide it. Gently slide the floss between your teeth, forming a C-shape around each tooth, and be sure to go below the gumline without forcing it. Use a clean section of floss for each tooth, including the back teeth, and finish by rinsing your mouth with water or mouthwash to remove any loosened particles. Aim to floss at least once a day, preferably before brushing, to maximize the benefits for your dental health.
