Case Presentation: Orthodontic Management of Recurrent Malocclusion with Ceramic Self-Ligating Brackets
Chief Complaint:
The patient presented with concerns about his uneven occlusion and the skewed appearance of his teeth. He also reported a history of repetitive relapse following previous orthodontic treatments.
History:
This is the patientβs third course of orthodontic treatment. He previously had a severe skeletal malocclusion that was addressed in earlier treatments. Despite these interventions, the patient continues to experience issues with dental alignment and occlusion.
Clinical Findings:
Upon examination, the patient exhibits:
1. Significant crowding in the mandibular anterior region.
2. Mild crowding in the maxillary anterior region.
3. Midline discrepancy, with the maxillary and mandibular midlines not coinciding.
4. Significant overbite and mild overjet.
Treatment Plan:
Given the patientβs preference for a less noticeable orthodontic solution, we opted for ceramic self-ligating brackets. Self-ligating brackets provide several advantages, including reduced friction, faster tooth movement, and shorter overall treatment duration. They also require fewer adjustments, enhancing patient comfort and convenience.
Treatment Objectives:
1. Alignment and Leveling: Resolve the crowding and achieve proper alignment of the dentition.
2. Midline Correction: Align the dental midlines to coincide with the facial midline.
3. Occlusion Improvement: Establish a Class I occlusion with an ideal overbite and overjet.
Expected Treatment Course:
A. Initial Phase:
Place ceramic self-ligating brackets on both the maxillary and mandibular arches. Use initial light archwires to begin alignment and leveling.
B. Intermediate Phase:
Progress to heavier archwires for further alignment and occlusal correction. Interproximal reduction (IPR) may be performed in the mandibular anterior region to alleviate crowding without extractions.
C. Space Creation:
Flare the canines as necessary to create space for the proper alignment of the opposing teeth.
Retention Plan:
After active treatment, retainers will be provided to maintain the achieved results and prevent relapse.
By addressing these orthodontic issues comprehensively and utilizing the benefits of self-ligating brackets, we aim to provide the patient with a stable, functional, and aesthetically pleasing outcome.
