UPDATED MAXICARE DENTAL APPOINTMENT PROCESS

1️⃣ Complete the Online Form
Fill out the Google Form with your complete and accurate details.

2️⃣ Review Your Benefits
Maxicare no longer verifies dental benefits for patients. Please check your coverage in advance. You may send us a copy of your benefit booklet for reference.

3️⃣ Book Your Appointment
Choose your preferred date and time and confirm your appointment.

4️⃣ Attend Your Consultation
Arrive on your scheduled date. The dentist will perform a consultation and assess your dental concerns.

5️⃣ Approval Before Treatment
After consultation, the clinic will request approval from Maxicare for the recommended procedures prior to treatment.
