Oroantral communication (OAC) treatment depends on the size of the opening and the presence of any sinus infection, ranging from simple conservative observation for tiny holes to surgical flap closure for larger defects.

Conservative and Medical ManagementSmall openings (2 mm or less): These often heal on their own with a figure-eight suture or gelatin sponge to stabilize the blood clot.

Medications: Antibiotics (such as amoxicillin-clavulanate) and nasal decongestant drops are prescribed to prevent sinus infection and keep air passages clear.

Sinus precautions: Patients must avoid nose-blowing, sneezing with a closed mouth, smoking, and using drinking straws for at least two weeks.

Surgical ManagementFlap techniques: Openings larger than 2–5 mm or chronic fistulas usually require surgical repair using local tissue flaps, such as a buccal advancement flap or a palatal rotation flap.

Buccal fat pad: The pedicled buccal fat pad flap can also be used to reliably close medium-to-large defects.Infection control: Any underlying maxillary sinusitis must be treated concurrently via drainage or specialized sinus surgery before or during closure.
