A female patient came in saying,
“Madalas dumugo ang gilagid ko at nahihirapan akong linisan itong fixed bridge ko.”

Clinical examination revealed a long-span fixed partial denture supported by teeth with compromised periodontal support. The diagnosis was generalized periodontitis, Stage III Grade C. A panoramic radiograph showed bone loss extending to the middle to apical third of the roots, indicating a poor long-term prognosis for the abutment teeth.

After a thorough discussion of the findings and treatment options, it was explained that the affected abutment teeth would need to be extracted due to inadequate bone support. Treatment was done in phases: extraction of compromised teeth, oral rehabilitation including cleaning and restoration of carious teeth, allowing time for healing and bone stabilization, and finally prosthodontic rehabilitation.

Removable dentures for both arches were provided for easier maintenance, better hygiene, and a design that is gentle on the supporting tissues.

Upon insertion, the patient shared:
“Grabe, parang nakayakap ang pustiso ko sa gilagid at ngala-ngala ko, Dokie. Mas comfy ito.”

This case highlights the importance of not neglecting the supporting structures of the teeth. While fixed prostheses can be a good option for properly selected cases, they are not ideal for everyone. Proper diagnosis and individualized treatment planning are essential.

Ultimately, treatment success is best seen in a patient’s comfort and satisfaction, which was evident in the photos taken. She looks more youthful and happier! 🤩🥰

We performed Non-Surgical Scaling and Root Planing (NSRP) to carefully remove deep, hardened deposits. After one week, the improvement was already noticeable. One month later, we re-evaluated her periodontal status and reinforced proper oral hygiene to maintain her results.
