This case presented with a significant loss of vertical dimension of occlusion due to the early loss of multiple posterior teeth. The reduced posterior support resulted in increased stress on the remaining anterior teeth, leading to fracture, excessive wear, and abrasion.

To restore the lost tooth structure and improve the patient’s function and aesthetics, porcelain-fused-to-metal (PFM) crowns were placed in the anterior region.

For the posterior areas, Thermosens dentures were fabricated for both the upper and lower arches. These restorations provided the necessary posterior support to help re-establish the patient’s vertical dimension of occlusion, improve mastication, and restore a more balanced and functional bite.

The final result was a more stable occlusion, improved function, and a significant improvement in the overall appearance of the patient’s smile.

To make the case even more complex, the lower left lateral incisor was locked lingually, while the upper right canine and first premolar had erupted in switched positions.

Treatment involved premolar extractions and retraction of the lower arch to correct the anterior crossbite. Through careful treatment planning and precise tooth movement, we achieved a more harmonious smile, improved alignment, corrected bite relationship, and better overall function.
