For our Maxicare patients, please be guided on the updated policy for dental approval and verification.

Kindly log in to your Maxicare Member Portal or email [email protected] to request a copy of the following:

• Dental Benefits
• Card Expiration Date
• Dental Availments / Utilization Record

Once received, please take clear screenshots or records of these details and send them to us as proof.

If possible, you may also contact a Maxicare agent directly via landline to confirm your coverage, and keep a record of the conversation for reference.

We appreciate your patience, as Maxicare has implemented a new procedure for processing dental availments.

Acceptable format example:

Coverage:
Dental Package: Standard
• Oral Prophylaxis: 2 sessions per year
• Desensitization: 2 teeth per year
• Permanent Filling: 2 teeth per year

Unlimited Procedures (based on plan coverage, 2–3 teeth per day):
• Oral Consultation / Examination
• Simple Tooth Extraction
• Temporary Filling
• Simple Repair & Adjustment of Dentures
• Recementation of Crowns, Bridges, Inlays, Onlays
• Palliative Treatment (mouth sores/blisters)
• Gum Treatment (inflammation/bleeding)
• Emergency Dental Treatment
• Oral Incision and Drainage

Note:Wisdom Tooth Surgery and Panoramic X-ray are not covered.
