🦷 Case Spotlight: When a Tooth Cannot Be Saved by Root Canal

A patient recently visited our clinic after being referred for a root canal treatment on an upper premolar tooth.

After a careful clinical examination and X-ray evaluation, we discovered that the problem was not a simple infection. The tooth was affected by internal resorption — a condition where the tooth structure starts breaking down from the inside.

In this case, there was already severe loss of the root tip (apical portion). Because of the extensive damage, performing a root canal would not provide a stable, long-term solution. The remaining tooth structure was too compromised to support treatment.

For the patient’s long-term oral health and to prevent further complications, extraction was the safest and most predictable option.

✅ The procedure was completed successfully
✅ Post-operative care instructions were given
✅ We are now discussing options for tooth replacement while the patient is under recovery.



🔎 What is Internal Resorption?
It is a rare condition where the inner part of the tooth starts to dissolve. It often progresses silently and may only be detected through X-rays.

Early diagnosis is key. Not all teeth recommended for root canal therapy are straightforward cases — proper evaluation makes all the difference.

Crown fractures caused by blunt trauma are among the most common dental injuries, particularly in children and adolescents, with a high prevalence in the upper front teeth (maxillary incisors). These injuries result from forces that exceed the elasticity of the tooth structure, often caused by falls, traffic accidents, sports injuries, or physical aggression.

For this case the upper lateral incisor fractured, the patient dived on a shallow swimming pool hitting the pool floor eventually.
