Patient came in for a dental implant since patient has missing 1st premolars. Upon checking, space available in the right and left side are not symmetrical, along with the natural rotation and angulation of the natural teeth, with that, we also observed the redness of the gum tissue in the upper incisors caused by the previous porcelain fused to metals. Patient wanted to change them as well for a better smile.

Patient chose to have a combination of crowns and bridge instead of dental implants.

This was a very challenging case since the 4 upper incisors are all made up of root canal treated and metal posts. Upon removing the previous crowns, preparation was a violated subgingival preparation, tooth was severely darkened due to metal posts and pigmentation of the old porcelain fused to metal crowns. We waited for almost a month and made 2 revisions of the provisional before taking a final impression to let the gums / soft tissue heal.

For the 4 incisors, we made a double coping zirconia and zirconia bridge on the right and left side 13-15, 23 - 25. Used opaque cement as well to make sure that the discolored tooth is fully masked.

After years of putting dental health aside, patient finally decided to jump the gun and wanted to fix everything that needs fixing. Most of all, patient wanted to change the removable dentures to a fixed bridge (zirconia) for more comfort and less maintenance, of course for a better smile. The upper arch was canted so the case was quite challenging but we hope we delivered. 🤪

Patient pre booked our appointment months in advanced for this smile transformation. Fortunately the patient provided us with pre op panoramic xray and photos before flying in from Canada.

We went for a natural look and used combination of zirconia crowns, bridges and Emax veneers (11 units). Patient looks like Mackenyu now LOL
