Fluoride Is Not an Upsell. It Is a Recommendation. Too often, fluoride recommendations disappear the moment insurance won’t pay. That is a financing decision, not a clinical one.

The ADA’s evidence-based guidelines recommend professionally applied fluoride for patients at elevated caries risk, regardless of insurance coverage. Research from Washington State found that reimbursement increased fluoride varnish use, but payment alone did not solve the problem. Communication did.

Patients are far less likely to object to paying than they are to recommendations that feel generic or sales driven.

There is a world of difference between:

“Would you like fluoride today?”

and

“Because your dry mouth and root exposure increase your risk for decay, I recommend fluoride varnish. Your insurance doesn’t cover it, and the fee is $35.”

One sounds like an add on. The other is a diagnosis followed by an evidence-based recommendation.

The answer is not to recommend fluoride to everyone. It is to recommend it consistently to the patients who will benefit most, explain why, disclose the cost, and let the patient decide.

Dentistry does not need better sales techniques. It needs better risk assessment and better communication.
