2. HEALTH PROTOCOLS. A personal health screening will be made to you before you can obtain an appointment for dental treatment. We beg you to BE HONEST in your health declarations as it could put us all in detriment if we fail to provide truthful health information. Rest assured, confidentiality of your statements are preserved.
