Many impacted wisdom teeth cause no pain, swelling, or obvious symptoms. However, that does not always mean they are harmless.

A 2024 study published in The Saudi Dental Journal found that impacted lower wisdom teeth were frequently associated with tooth decay, particularly on the back surface of the neighboring second molar. In many cases, the healthy tooth next to the wisdom tooth was affected more often than the wisdom tooth itself.

Researchers analyzed 1,591 dental panoramic X-rays and found that nearly 40% of impacted lower wisdom teeth were associated with carious lesions. The highest risk was seen when the wisdom tooth was tilted forward toward the adjacent tooth (mesioangular impaction), creating an area where plaque, bacteria, and food debris can accumulate and become difficult to clean.

This is one reason dentists often recommend regular monitoring of impacted wisdom teeth. Hidden decay can develop between teeth long before pain appears, and by the time symptoms occur, significant damage may already have occurred.

Regular dental examinations and panoramic X-rays can help detect these problems early and protect the healthy tooth next to an impacted wisdom tooth.

Ever wondered how dentists pulled teeth before modern dental tools existed?

Meet the Dental Pelicanβ€”one of the earliest specialized tooth extraction instruments in history.

Believed to have originated in 14th-century Europe, the Dental Pelican remained in widespread use for centuries before modern extraction forceps were introduced. Named after its resemblance to a pelican's beak, it worked by using leverage to loosen and remove diseased teeth.

Compared with today's precise dental instruments, the Dental Pelican was far more traumatic and often damaged the surrounding gums or bone. Yet for hundreds of years, it represented one of the best tooth extraction methods available.

Its story reflects how dramatically dentistry has evolvedβ€”from crude mechanical tools to safe, precise, and comfortable treatments supported by modern science.

This is why even a small cavity should never be ignored.

A cavity may look small on the surface, but that doesn't always reflect the amount of damage hidden inside the tooth.

Unlike enamel, the dentin beneath it is softer and less mineralized, allowing tooth decay to spread more quickly once bacteria break through the outer protective layer. Because this process often occurs silently, many cavities continue to grow without causing pain or obvious symptoms.

By the time a tooth starts to hurt, the decay may have already reached the pulpβ€”the living tissue that contains nerves and blood vessels. At that stage, treatment is often more complex and may require root canal treatment or, in severe cases, tooth extraction.

The good news is that most cavities are highly treatable when detected early. Regular dental checkups and timely treatment can preserve more of the natural tooth, reduce treatment costs, and help prevent avoidable complications.
