TODAY IS WORLD ORAL HEALTH DAY!

A happy mouth is a happy life!

Dentists love what we can see, measure, prep, cure, fill, bond, seat, adjust, and radiograph. If it is not on a film or lighting up on a CBCT, we get suspicious. That is why placebo and nocebo talk sounds fluffy, but the evidence says it is real, measurable, and clinically relevant.

Dentistry is one of the best research models for placebo and nocebo effects because third-molar surgery produces predictable pain in healthy patients. That makes it ideal for studying how expectation, communication, and biology interact.

Placebo means positive expectation improves the experience.
Nocebo means negative expectation makes it worse.

Studies show placebo patients report side effects at rates similar to real drugs, meaning expectation alone can cause nausea, dizziness, and more pain. Dentistry is full of nocebo triggers like needles, drills, surgery, and bad memories, so what we say matters.

This does not mean pain is imaginary. Biology matters. Technique matters. But every procedure has two effects. What you do, and how the patient experiences it.

Tone, confidence, wording, and trust can change the same procedure from easy to awful.

So here is the real question.

How much of your patient’s pain is coming from your handpiece, and how much is coming from your words?

Most of us were not lied to about endodontics. We were taught simplified rules that worked most of the time, and then we forgot they were simplifications.

Root canals do not sterilize teeth. They reduce bacterial load and rely on a good seal and a healthy host response. Radiographic healing does not automatically equal biologic cure. A J shaped radiolucency is not a guaranteed vertical root fracture. A beautiful obturation does not protect a tooth with no ferrule.

In real life, structural integrity and restoration quality often determine survival more than the endodontic technique itself. Many “failed root canals” are actually biomechanical failures. Many “implant wins” are simply better case selection.

The biggest distortion in private practice is believing endodontic success lives inside the canal. It usually lives in the restoration, the occlusion, and the cracks you cannot see.

If we slow down, stack diagnostic clues instead of reacting to one sign, and stop treating endo versus implant like a philosophy, our decisions improve.

What belief about root canals do you think causes the most unnecessary extractions or unnecessary heroics in everyday practice?
