Most of us were not lied to about endodontics. We were taught simplified rules that worked most of the time, and then we forgot they were simplifications.

Root canals do not sterilize teeth. They reduce bacterial load and rely on a good seal and a healthy host response. Radiographic healing does not automatically equal biologic cure. A J shaped radiolucency is not a guaranteed vertical root fracture. A beautiful obturation does not protect a tooth with no ferrule.

In real life, structural integrity and restoration quality often determine survival more than the endodontic technique itself. Many “failed root canals” are actually biomechanical failures. Many “implant wins” are simply better case selection.

The biggest distortion in private practice is believing endodontic success lives inside the canal. It usually lives in the restoration, the occlusion, and the cracks you cannot see.

If we slow down, stack diagnostic clues instead of reacting to one sign, and stop treating endo versus implant like a philosophy, our decisions improve.
