🦷✨ Another Surgical Case at Tesoro Dental Clinic ✨

Today, we managed another surgical procedure for a patient referred to our clinic by a colleague.

The patient presented with a tissue overgrowth at the site of a previously extracted upper first molar.

Radiographic evaluation showed close proximity to the maxillary sinus, so a careful and conservative approach was planned.

The lesion was excised from its base using laser, allowing better precision and reduced bleeding during the procedure.

The excised tissue was sent to the hospital for histopathologic (biopsy) examination.

We remain hopeful for a negative and benign result.

Cases like this highlight the importance of proper evaluation, referral, and gentle surgical care—always with patient safety as our priority. 💙

Today, we performed a two-stage wisdom tooth procedure for a deep, horizontally impacted third molar that was very close to the inferior alveolar nerve (IAN).

Using a piezo surgical machine, we carefully removed only the crown of the tooth during the first stage to protect the nerve and avoid complications.

The roots were intentionally left in place and will be re-evaluated after a few months before considering the second stage.

This careful, step-by-step approach ensures the safest possible outcome for our patient.

Because when it comes to surgery, playing it safe is always the best choice.

Close your teeth right now. Which side hits first?

If you feel more pressure on one side, your spine is compensating at this exact moment.

Here’s the cascade destroying your posture work:

The jaw drags your skull off-center. When your bite lands harder on one side, your jaw pulls your entire skull with it. Your head tilts. One ear drops. Your neck muscles grip all day just to keep your eyes level.

The crooked head forces your spine to bend. Your brain won’t let you walk around tilted, so it bends and rotates your spine. One shoulder climbs. Ribs twist. That one-sided tightness you keep foam rolling but can never fix.

When is a 2-Stage Surgery Approach Done for Impacted Tooth Removal?

A two-stage surgical approach is recommended when removing an impacted tooth poses a high risk of damaging the inferior alveolar nerve (IAN) or nearby vital structures. It is typically chosen when:

🔹 The roots of the impacted tooth are very close to or encroaching on the IAN canal (seen in radiographs or CBCT).
🔹 There is a high chance of nerve injury if removed in a single procedure.
🔹 The tooth is deeply impacted and full removal may cause excessive trauma.
🔹 Coronectomy is preferred initially to allow the roots to migrate away safely before final removal.
🔹 The patient presents with anatomical variations that increase surgical difficulty.
🔹 Clinician aims to minimize post-operative risks such as paresthesia, dysesthesia, or nerve damage.

In short, 2-stage surgery is chosen to prioritize patient safety when a single-stage extraction carries too much risk.
