Ever seen C shape canal configuration on an upper molar?

Canals can communicate between M and D canals, M and Pa canals, D and Pa canals and occasionally all three.

Common mistake I frequently see is missed second buccal canal when roots are fused.

In this case, the distal canal was detected 2 mm inside the mesial canal. While, mesial canal joins with the palatal canal.

My go to files for isthmuses are always FKG XP Shaper and XP finisher. Highly useful files for C shape canals.

Will review the patient again in 6 months.

Sharing a very embarrassing case with some learning points — a patient was referred for management of a separated file, and I ended up referring the case back with an additional separated file.

At presentation, there was already a separated instrument in the MB2 canal. After successfully bypassing the fragment, I unfortunately separated another instrument right at the end of canal instrumentation (the blue arrow) - a true “super suey” moment. It was during the busy pre–Chinese New Year period and, admittedly, i got a little impatient.

I was prepared to take out my Terauchi tip but after some thought, retrieval may not be necessary and would likely result in unnecessary dentine sacrifice. So, I chose to follow up.

At the 3-month review, there was complete bone formation and resolution of the periapical mucositis.

Cases like this reinforce an important principle in endodontics: successful outcomes are driven primarily by effective canal disinfection.

Key reminders:

1) Always use rubber dam. Take time to disinfect the canals. Don't rush.

2) Make decisions based on biological objectives, not procedural ego or radiographic perfection.

3) Be accountable. Inform patients honestly whenever instrument breaks. Let your indemnity deal with the patient.

When it's difficult to achieve patency in distal canals of lower molars, always consider the possibility of a sharply curved apical exit—often towards the distal.

In root canal treatment, every millimeter matters (Ng et al., 2011). As advised, if you are stuck, please don't force your way. You saw how my fingers moved - no force involved.

Sometimes, the body heals in ways we still cannot fully explain.

This patient with an endo-perio lesion on tooth 37 had severe bone loss extending all the way to the apex, making the prognosis quite questionable. With such extensive destruction, the blood supply is often compromised, and we were unsure if the tooth could even be saved. He was supposed to be reviewed by periodontist Dr. Tan Sze Jun, but defaulted treatment for almost two years.

When he finally returned for review, an OPG was taken to reassess his overall periodontal condition—and to our surprise, the bone showed remarkable healing without any intervention.

Still not sure what magic happened there, but cases like this remind us that the human body can sometimes be the best healer of all.

Hi Thermafil obturation system… long time no see.
Not gonna lie — I didn’t miss you.

Back in the day, this “legendary” technique had a solid fan base:
A plastic carrier + oven-softened Gutta-percha = send it down the canal and it will flow into all sorts of nooks and crannies via hydraulic pressure.

It became the go-to move when:
• Patency said “no”
• Ledges said “try again next life”
• Canal anatomy looked like abstract art

And the logic?
“Maybe the GP will flow there…”
“Maybe it will seal everything…”
“Maybe… miracles exist…”

Fast forward to today — same movie, new cast.
Enter: Bioceramic sealer.

Now it’s:
“Don’t worry, the sealer will take care of it.”

But let’s be honest…
Bacteria don’t care how fancy your material is.

You can’t seal in what you didn’t clean out. You can’t obturate your way out of poor debridement

Endodontics isn’t:
“Fill and pray”

It’s:
Clean. Shape. THEN fill.

Because at the end of the day…
Bacteria always win — unless you remove them first.

P/S : Fill and Pray is something I did a lot during my postgraduate days. Dr Khiratti oughta have seen that face a lot in the x-ray room - when my nurse was fixing and developing radiographs - yet probably another archaic witchcraft.

A J-shaped lesion is not always indicative of a root fracture.

In this case, endodontic microsurgery was performed to rule out crack. The MB1, MB2, and the isthmus were then thoroughly cleaned and sealed.

6-month review demonstrates very encouraging bone healing - the periapical bone has almost completely reformed.

Looking forward to reassessing this case again at the 12-month mark.
