Putty Index (Part 2)

A continuation to dental tip from the video last week, in some class IV cases we are often faced with time constraint in executing the treatment which can be due to factors like emergency situation, lack of resources or armamentarium which leads to having a diagnostic wax up of the intended final restoration is impossible to be produced.

An alternative to this, instead of preparing a wax up or build up on the tooth prior to creating the putty index, this method could come in handy. In this situation a silicone putty impression of the fractured tooth and the record made on the fractured portion is trimmed off to replicate the palatal inclination of the adjacent central incisor.

The index putty can be used as per normal palatal key once the excess has been trimmed off and you can start place the first layering of your composites.

Putty Index
For this week’s tip, we are looking at the popular usage of putty of polyvinyl siloxane elastomer material (Aquasil soft putty/ regular set, Densply was used in this video) for management of Class IV restoration.

There are several reasons why polyvinyl siloxane elastomer is being used as the silicone key or putty index, for example, its ability to facilitate in optimizing adhesion of composite to mineralised hard tissues, enhances marginal adaptation, seal and mechanical strength and reduces microleakage and increases retention are among the many reasons of it being the perfect impression material to be used for the silicone key. (Nalbandian S., 2009)

It is important for the clinician or the assistant to mix well the base and the catalyst of the putty into a homogenous colour and it was reported that usage of latex gloves containing sulphur or sulphur derivatives during the mixing would delay or inhibit the PVS setting time.(Niellsen et al, 1986)(Rosen M et al 1989)(Ravikumar C.M et al 2012). Hand contaminated operator from the residual powder after degloving may also affect the setting time of these materials (Matis B.A et al,1997)(Delgado A.J et al 2018).

Hence, mixing with clean bare hands are the best guys!

Since its major innovation in the year 1864 by a New York dentist, Dr Sanford Barnum, clinicians had been using rubberdams with different techniques to reach the same goals of isolation. One of the techniques is the split dam technique.

Popular for its usage when attempting to isolate a badly broken down tooth, during the cementing of veneers or crowns, in restorations with subgingival margins among others, this technique has been around and practised for many years.

The split of the rubberdam may not give ideal isolation and clinicians may need to use additional caulking agent, putty silicone for the palatal or liquid dam to seal any defective areas.

For the first sharing session of the dental tips this week, we are looking at methods of rubberdam inversion.

Inversion of the rubberdam is needed especially when a good moisture isolation, a dry working field and clearer view of the margins of the restorations are needed. Besides the advantage of holding the rubberdam snugly into the cervical area of the tooth, it plays a role in cases where placement of clamps might not be possible.

Knot ligatures with double knots dental floss is one of the many ways on inverting the rubberdam.

The floss will be tightened as we ligate it around the cervical of the tooth.
