On and off toothache occur on lower right second molar since 2years ago. Xray found that the previous filling was near to the pulp horn and there was big lesion extending from the tip of the root. Root Canal Treatment (RCT) was carried out. Distal C-shape canal and mesial canal were cleaned and filled with MTA.

When a fistula/swelling persists after standard root canal treatment, Apicocectomy is the option to remove infection and retro-fill the root.
During the procedure, the tip of the root and infection tissue were removed. Then the tip of root was filled with MTA. The crown still remained intact.

At 2 months review, it shows no more swelling at the gum and XRAY shows increased of radio-opacity(healing) at the defect area👍🏻
