*** Enamel Infiltration ***
This moderate MIH case was treated using conscious home-bleaching with 5% carbamide peroxide (Flairesse, DMG) for 14 days.
A flowable composite was used for the retention of the rubberdam. After one etching cycle using hydrochloric acid (Icon Etching, DMG), transillumination was used after application of ethanol (Icon Prime, DMG). The large contrast suggests additional acid-erosion and after, in total 4 cycles, this changes to a more favorable transillumination outcome. Infiltration was done for 20 minutes (Icon Infiltrant, DMG) and successfully removed the discolorations present.
The infiltrated enamel was pretreated using air-abrasion (AquaCare, Velopex) phosphoric acid etching (Ultra-Etch, Ultradent) and bonding (Optibond FL, Kerr) prior to application of composite (Estelite Asteria, Tokuyama). Final finish and polish was done with diamond impregnated wheels (Diacomp Twist, EVE).
In a separate session the lower jaw was also treated.
Infiltration has become more and more predictable, but it took me years of study, training and managing failures to reach a higher level.
