Case 17: Class III, Facial Asymmetry & Retained Baby Teeth

The patient presented with concerns about her crooked teeth and facial asymmetry. She had a severe skeletal Class III malocclusion with congenitally missing lower premolars (34, 35 and 44), resulting in retained baby teeth (63, 74, 75 and 84). Her lower jaw was deviated to the left, and the upper left canine had become significantly overerupted.

Treatment involved fixed braces, removal of the retained baby teeth, and lower jaw orthognathic surgery (BSSO) to correct the jaw discrepancy and facial asymmetry.

The results speak for themselves, with a dramatic improvement in facial symmetry, smile aesthetics, and bite function. Best of all, the patient was thrilled with her new smile and is now ready for dental implants to replace her missing teeth.

Case 16: Class III, Facial Asymmetry, 19 mm Anterior Crossbite

This was a particularly memorable case, as it remains the largest anterior crossbite I have treated to date. While managing 5 to 12 mm anterior crossbites is not uncommon in orthognathic practice, an anterior crossbite approaching 2 cm is certainly a rarity.

The patient presented with concerns regarding both chewing function and smile aesthetics. Clinical examination revealed a severe skeletal Class III malocclusion with a 19 mm anterior crossbite and mandibular asymmetry towards the left side.

Our treatment plan involved fixed braces to prepare the arches for surgery. This was followed by Upper and lower jaw orthognathic surgery, with the maxilla advanced and downgrafted to improve smile aesthetics, while the mandible was repositioned posteriorly (with the airway in mind) and centred to correct the facial asymmetry.

The transformation was remarkable. Note the significant improvement in facial symmetry, facial profile, smile aesthetics, and occlusal function at the completion of treatment. Most importantly, the patient was absolutely thrilled with his new smile, facial appearance, and stable bite.

Case 15: Class III, Facial Asymmetry, Crowding & Anterior Open Bite

This 19-year-old patient presented with concerns about his unaesthetic smile. He had a mild Class III underbite, significant crowding, an anterior open bite, and was unable to bite properly.

Our treatment plan involved the extraction of two upper premolars. Presurgical orthodontics was carried out to align and level the teeth, relieve the crowding, and correct the dental midlines. This was followed by bimaxillary surgery, with the upper jaw moved forwards and downwards to improve smile aesthetics, while the lower jaw was repositioned slightly inwards and centred to correct the facial asymmetry.

Note the improvement in both the facial asymmetry and facial profile at the end of treatment. The surgery was planned with the pharyngeal airway in mind, ensuring that the lower jaw movement would not compromise the airway despite being moved inwards. In fact, the postoperative airway was wider than before treatment.

He was absolutely thrilled with his new facial profile, smile, and stable bite!

Case 14: Jaw surgery - Class III, Facial Asymmetry, Occlusal Cant (tilted smile)

This 19-year-old patient presented with concerns about his asymmetrical appearance. He had a mild Class III bite and a significantly tilted smile.

To achieve an ideal result while minimizing risk and cost for the patient, we chose a combined approach rather than traditional double-jaw surgery:

🔹 Orthodontics: Used to align the teeth and correct the upper jaw cant non-surgically.
🔹 Orthognathic Surgery: Limited to the lower jaw to correct the skeletal asymmetry.

By managing the upper arch orthodontically, we significantly reduced the surgical complexity, risk, and cost while delivering a flawless, balanced result. He was absolutely thrilled with his new facial profile and stable bite!

Case 13: MARPE / MSE

Bilateral crossbites in adults can be challenging to manage, as the maxillary sutures are no longer easily expanded after the late teenage years. Traditionally, severe cases often required jaw surgery to widen the upper jaw.

Today, with MARPE / MSE (Maxillary Skeletal Expansion), we are able to achieve expansion at a skeletal level — often avoiding the need for surgery.

With MARPE / MSE, we can:
✔️ Expand the upper jaw skeletally (not just tipping teeth)
✔️ Create space for proper alignment
✔️ Improve bite coordination
✔️ Potentially enhance airway dimensions

Following expansion, orthodontic treatment with fixed braces or clear aligners is used to align the teeth and close residual spaces for a stable, functional result.
