Gingival Recession Treatment – 3-Year Stability: One of the most common questions patients ask is: “Will the graft last?”

Research shows that mucogingival surgery can provide predictable root coverage and increased tissue thickness, with studies reporting stable outcomes even after 5–10 years (Carbone et al., 2024).

This case shows multiple gingival recessions affecting the upper and lower right teeth, treated with mucogingival surgery.

Graft: Autogenous de-epithelialized FGG (CTG)
Root conditioning: PrefGel® ( )
Donor site care: Collagen sponge + Periacryl ( )

📅 3-year follow-up:
• Stable gingival margins
• Healthy soft tissue 
• Increased soft tissue thickness

The key to long-term stability is phenotype modification — thicker tissue is more resistant to future recession.

Periodontitis is one of the common causes of gingival recession.
Following stabilization of the periodontal condition, root coverage surgery may be considered to address the recession.

In this case, buccal bone loss, a prominent root, and mild root rotation added to the clinical challenges.
A conventional approach using CAF + CTG was performed, aiming for a predictable and stable outcome.

🦷 Donor site care: Blue®M gel ( .malaysia )
🦷 Recipient site care: Gengigel® mouthrinse twice daily for 4 weeks (.my )

Tooth #35 presented with a perio-endo lesion and recurrent periodontal abscess, leading to extraction.
CBCT after 6 months showed buccal and crestal bone loss.
Implant placement was done with simultaneous GBR.
At 4 months post-op, bone regeneration was satisfactory, but buccal soft tissue deficiency remained. A free gingival graft (FGG) was performed to improve the soft tissue phenotype.
Final screw-retained implant crown was delivered after another 3 months.

Grateful for the healing and the final outcome. Each case is a reminder to me that implant dentistry is always a learning journey.

Excisional Biopsy –
Complete lesion removal is essential for accurate diagnosis and to reduce the risk of recurrence. Histopathologic examination remains the gold standard for definitive diagnosis.

✔️ Deep incision to bone to ensure complete excision
✔️ Removal of the palatal papilla to eliminate residual lesion on the palatal surface
✔️ Elimination of local contributing factors: subgingival calculus on the proximal surfaces of teeth 23 and 24, removed with thorough deep scaling and debridement
✨Oral hygiene demonstrations - ensure good healing and reduce risk of recurrence

💡 Discussion point:
A crack line was observed on tooth 24. The patient is asymptomatic and pulp sensibility is normal. Could this have contributed to the gingival overgrowth?

A quick review of a soft tissue graft before my Christmas and New Year break begins. Wishing everyone a Merry Christmas 🎄 and a Happy New Year 2026 filled with health and smiles! 😊

Case: Deep isolated mandibular gingival recession - Before and 4 weeks after surgery ✨

Causes / risk factors :
* Thin gingival phenotype – limited buccal bone & thin keratinized tissue
* Poor plaque control → inflammation
* Prominent root 

Surgical Treatment:
* Laterally Closed Tunneling technique (Sculean & Allen, 2018) + connective tissue graft (CTG) 
* Palate donor site managed with BlueM gel, collagen sponge, and sutured with nylon & Periacryl 
*Post-op care: Gengigel mouthwash for 2 weeks .my

Healing Timeline:
* Week 1: Stable flap, mild swelling, normal early healing
* Week 2: Inflammation decreases as the graft integrates, and root coverage remains stable with improved tissue firmness.
* Week 4: Stable, healthy soft tissue adaptation with near-complete root coverage of the lower incisor.

Periodontal regeneration is usually performed with bone grafts plus a membrane or Emdogain. In this case, the site had thin tissue and limited keratinized gingiva, so we included a connective tissue graft (CTG) alongside the procedure.

Three months post-op, the healing looks encouraging, and the donor site has healed well. The tooth still tests vital, and the patient reports no discomfort. Grateful to see how careful soft-tissue management can support good regeneration outcomes.
