Lower Molar Implant — From Healing to Final Crown

Restoring a missing lower molar is not simply about placing an implant — the goal is to rebuild a tooth that looks, feels, and functions naturally.

This case shows the restorative journey:

1. Implant healing phase — allowing stable integration with the surrounding bone and soft tissue.
2. Healing abutment removed — revealing the established soft-tissue emergence profile.
3. Screw-retained zirconia crown fitted — designed for strength, cleansability, and retrievability.
4. Final restoration — occlusion carefully checked and adjusted before completion.

For posterior implants, good treatment planning involves more than achieving implant stability. Implant position, soft-tissue management, emergence profile, proximal contacts and occlusion all contribute to a predictable long-term result.

When an old filling starts to crack, there may be more happening underneath.

This patient presented with a cracked old amalgam restoration. After removing the amalgam, we discovered secondary caries together with a crack line extending through the remaining tooth structure.

All defective and carious tooth structure was carefully removed and the tooth was assessed before proceeding with definitive restoration.

Due to the extent of structural compromise, a direct filling was no longer the ideal option. We restored the tooth with a zirconia crown, providing full coverage to protect the remaining tooth structure and restore its function.

What we found:
Old amalgam restoration → Secondary caries + tooth crack → Defect removal → Zirconia crown

Sometimes, replacing an old filling isn’t simply about replacing the filling — it’s about identifying what is happening underneath and choosing the appropriate restoration for the remaining tooth.

Treatment decisions vary according to the extent and location of the crack, remaining tooth structure, pulpal status, and individual clinical findings.

This patient came to us with severely decayed upper front teeth. Instead of removing them and replacing them with implants, we performed root canal treatment and restored the teeth with crowns—bringing back both function and aesthetics.

Even as an implantologist in Ipoh, our core belief remains the same: preserve natural teeth whenever they can be predictably restored. Dental implants are an excellent solution for teeth that cannot be saved, but they should not replace treatable natural teeth.

Not every cervical abrasion needs aggressive treatment.

In this case, the worn cervical lesions were restored with direct composite resin, rebuilding the lost tooth structure while preserving as much healthy tooth as possible.
