A patient from KL came to our clinic due to persistent pain after the first visit of root canal treatment in another clinic.

Upon taking an X-ray, Dr Tan Well Soon noticed something unusual. Therefore, a CBCT scan was taken for a more thorough examination. A rare anatomical morphology was noted. The complaint tooth was presented with a C-shaped root (an upper first molar with a C-shaped root).

With inadequate visibility, variations in tooth morphology can make it difficult for dentists to locate the canal orifices and may lead to iatrogenic perforation.

The tooth was treated under a dental microscope. 🔬All four canals (MB1, MB2, DB and P) were negotiated to their full working lengths. The perforation was repaired with MTA. Granulation tissue at the perforation site was removed using a laser before the perforation was repaired with MTA.

The symptoms resolved on the same night. 🦷👌🏻The patient remained asymptomatic for two weeks, and obturation was completed after two visits.

The prevalence of a C-shaped root or canal in an upper (maxillary) first molar is low, ranging globally from approximately 0.8% to 2%, depending on the diagnostic method and population studied.

Pain after root canal? Sometimes the problem isn’t the treatment—it’s a hidden canal that was never found.💫🦷

A patient was referred to Dr Tan Well Soon due to persistent pain following a previous root canal treatment.

After a CBCT scan was taken, Dr Tan Well Soon identified a missed canal that had not been treated. The scan revealed a deep apical split, where a single root canal divides into two separate canals near the root tip (apex). Studies have shown that lower first premolars can have two canals at the apex in approximately 24%–25.5% of cases.

With the help of CBCT x-ray and dental microscope, these hidden canals can be located and treated with greater precision, helping to eliminate infection, relieve pain, preserve the natural tooth, and improve the long-term success of root canal treatment.
