What happens when teeth start to fail?

This patient came to us with a mobile upper anterior bridge. The lack of posterior support on the left side had contributed to excessive loading on the anterior bridge, compromising its stability.

After extracting the failing teeth, we allowed the sockets to heal for eight weeks while the patient wore a provisional denture.

We then proceeded with implant rehabilitation:
Anterior region: Osstem TSIII implants were placed to replace the missing teeth.
Posterior region: Osstem SS implants were placed to re-establish posterior support, with simultaneous bone grafting to improve the bone contour.

Beyond replacing the missing teeth, we also managed to correct the crossbite involving the upper left canine and improve the alignment of the canted upper incisors.

The goal wasn't simply to replace failing teeth, but to restore balance, function and aesthetics. By addressing the underlying lack of posterior support alongside the anterior concerns, we could work towards a more stable and maintainable long-term outcome.

Sometimes, the key to a good restoration is creating the right space before we start restoring.
This patient wanted to reduce the protrusion of her front teeth. However, she was also missing multiple posterior teeth and had very limited chewing function.
On clinical examination, there was virtually no restorative space available in the posterior region. Rather than simply replacing the missing teeth, we first needed to create the space required for a proper functional restoration.
How we tackled this case:
Step 1: Establish the restorative space
Interim dentures were used to increase and re-establish the desired restorative space.
Step 2: Reposition the anterior teeth
Fixed appliances were placed on both the upper and lower arches, with TADs used to facilitate retraction of the upper anterior teeth. This allowed us to reduce the overjet and overbite while creating sufficient space for the posterior restorations.
Step 3: Replace the missing teeth
Once the anterior teeth had reached their ideal positions, Osstem tissue-level and bone-level implants were planned and placed to replace the missing posterior teeth.
Step 4: Definitive restoration
The implants were restored, completing the posterior support and allowing the patient to discontinue the use of her dentures.
By combining orthodontics, implant placement and restorative treatment, we were able to address both the aesthetic and functional concerns. With her posterior teeth restored and her anterior teeth repositioned, the patient finished with a more balanced smile and significantly improved chewing function. We are incredibly fortunate to have a patient who was willing to patiently undergo the various stages required to establish a solid foundation for implant treatment.
He initially presented with a severely atrophic maxilla resulting from the failure of a tooth-supported bridge. A titanium-reinforced PTFE membrane was used to maintain space and support new bone formation during the healing process.
To facilitate easier long-term maintenance, two separate three-unit implant bridges were planned for the anterior region. Osstem TS IV implants were selected for the anterior segment because of their tapered design, while Osstem SSIII implants were placed in the posterior region to provide improved soft tissue support.

The lovely patient presented to our clinic seeking to restore her upper right posterior teeth. Due to severe bone loss in the molar region, only 1 mm of residual bone remained beneath the maxillary sinus.
To simplify treatment and significantly reduce the overall treatment time, two Osstem Bone Level implants were placed in the premolar positions using the fully guided OneCAS Kit. This approach avoided the need for extensive bone grafting and sinus augmentation. The replacement molar was designed as a slightly reduced pontic, providing a functional and aesthetic outcome while minimising the need for additional surgery.
As a result, the patient's implant bridge was delivered in just three months, compared with the approximately nine months typically required when bone augmentation procedures are performed. This streamlined approach reduced treatment complexity, shortened the rehabilitation period, and improved patient acceptance without compromising the final outcome. Case update

This patient had extractions done with immediate implant placement simultaneous bone augmentation done. An Osstem TSIII implant was used in this case.
During the restoration phase, often times, adjustments can be made to improve the outcome further. In this case, the bridge seemed rotated during try in because of the uneven incisal edges. Pink porcelain was added and the incisal edge was adjusted to give patient a more balanced smile. A fully guided implant protocol makes the treatment more efficient, turning what used to be a stressful longer procedure, into a simple straightforward routine. Giving the patient a pleasant experience as routine as getting a filling done. This upper premolar was planned for extraction, however, immediate implant placement was avoided due to the close proximity of the sinus. Ridge preservation was done, and a tissue-level Osstem SSIII 4.0x7mm implant was placed. A surgical guide provided a definite stop, avoiding the sinus and a more complicated sinus lift procedure. Single anterior tooth replacement can be challenging to restore. This patient had a missing tooth, with inadequate bone volume, asymmetrical width of the right incisor the tooth restored, deep bite. Orthodontic treatment was offered but the patient preferred a restorative only approach. An Osstem TSIV implant was used in this case as a tapered implant was more suitable for this case. The bone was augmented at the time of implant placement. The gum tissue was adequately thick, so a screw-retained implant crown was issued at 3 months after implant placement. When two teeth are missing, a cantilever implant bridge is a good option. The space can be restored with natural looking teeth without the extra cost of a second implant. Here, an Osstem SS 4.1x8mm implant was placed, and restored at 3 months with a nicely fabricated bridge.

The patient wished to replace a missing premolar; however, a large buccal bone defect was present as the tooth had been extracted a long time ago. Simultaneous implant placement and guided bone regeneration were carried out using an Osstem SS implant and a xenograft bone substitute.
