In this case, the first molar was failing due to a missed MB2 canal.
1️⃣ Before even touching the old Gutta Percha, I performed a Pre-endodontic Build-up and Immediate Dentin Sealing (IDS). I wanted to seal the dentin and build my walls before using Gutta Percha solvents. This ensures that the solvents don’t contaminate the hybrid layer, preserving the highest possible bond strength for the final restoration.
2️⃣ I negotiated the missed MB2 canal. Using a warm vertical condensation technique ensures a dense, 3D obturation of the entire root canal system.
3️⃣ Endodontically treated teeth are prone to fracture. That’s why I integrated Ribbond fibers into the core of the restoration.

To ensure the teeth remained vital, we used Caries Dye to differentiate between infected dentin (which must go) and affected dentin (which can stay). This objective approach allows for maximum tooth structure preservation near the pulp.
Restoring back-to-back Class II cavities is one of the toughest anatomical challenges. We utilized two metal sectorial matrices face-to-face to recreate the natural emergence profile of both teeth simultaneously, resulting in a tight, cleansable contact point that won’t trap food.
This protocol resulted in two vital teeth, zero pain, and a perfect anatomical reset.

Restoring a fractured incisor on a young patient presents a unique challenge: partially erupted teeth. Traditional clamps are often impossible to place, but Isolation is Non-Negotiable.
Since these teeth aren’t fully erupted, I utilized double dental floss ligatures to secure the rubber dam. This technique helps me to make the young little girl accepts using rubber dam.
Always I do 30° Beveling on all my restorations to make them disappear so the eye can’t differentiate where my composite begins.
This tooth was restored by a layering protocol:
• Opaque Dentin increments to provide the core structure.
• Transition Zone management to blend the composite seamlessly.
• The “Halo Effect”: Recreating that youthful incisal translucency that makes a restoration look alive.

Restoring multiple posterior teeth isn’t just about “filling holes”, it’s about rebuilding a functional unit.
📍 Session 1 (Premolars) :
We started with the 1st and 2nd premolars. Under strict rubber dam isolation, we removed the infected tissue and focused on direct composite restorations. By isolating, we ensure the bond strength is never compromised by moisture.
📍 Session 2 (Molars) :
The second session was dedicated to the molars.
Using a sectorial matrix system with a tension ring. This allows us to recreate that tight, natural contact point and the perfect emergence profile.

I’m not sharing a before-after photo. I’m sharing a story where saving a tooth was the only option I desire.
From a structurally compromised tooth to functional harmony. I highlights on my post the power of combining high-level Endodontics with modern adhesive dentistry to save what others might extract.
1️⃣ All three teeth received endodontic retreatments. We utilized Bioceramic sealers with a warm vertical condensation technique for a true 3D obturation. Clean canals = Long-term success.
2️⃣ Reinforcement with Ribbond: To enhance fracture toughness and mimic the dentin-enamel complex, we integrated Ribbond fibers. This prevents crack propagation which is crucial for endodontically treated teeth.
3️⃣ The 1st Premolar Challenge :
Deep subgingival margins are the enemy of adhesion.
We elevated the margin using a strict rubber dam isolation protocol to move the restorative interface supra-gingivally.
A Lithium Disilicate (E.max) indirect restoration was chosen for the 1st premolar to provide maximum strength and esthetics.
4️⃣ Direct Biomimetic Composites:
The remaining two teeth were restored using direct biomimetic composite layering, focusing on anatomy and natural light reflection.
