This case highlights the progression and evolution of dental implant design and loading protocols over two decades, showcasing advancements in technology and techniques that improve patient outcomes and reduce treatment times. It is a typical case in private practice. It started in 1999 with a Tissue Level implant to replace missing tooth #36. While Conventional dentistry work is getting old and fails for this patient , Implant dentistry is showing long term outcomes, up to 25 years.

Honored to have shared with Dr. Paolo Casentini the closing session on “Immediacy” at the ITI M.E. Section’s annual Educational day. We debated the Immediacy in Implant Dentistry where new Implant design and digital technology are a big help. The take home message was that “Immediacy” shouldn’t be a clinical indication by default as shown in the mass media but rather a case selection, where patient information is key.

Among few worldwide 3-years follow-up X-rays confirming healthy and stable bone around an immediately loaded Straumann® BLX 4.5mm D. SLActive 10mm L. implant.
It was placed in a healed socket #46 and restored prosthetically 4 days after implant placement through a hybrid workflow for the temporary phase and 6 weeks after, for the final restoration with monolithic zirconia crown on Variobase abutment..

11 years follow up of multiple teeth replacements in the anterior region. Early implant placement Type 2 of 4 BL RC implants at #13, #11, #21 & #23 site.
A Temporary FPD was inserted, 3 months after Implants placement, for soft tissue management.
3 months later, the final prostheses were placed. The photos & Xrays, up to 11 years, show the evolution and soft and hard tissues stability around the implants .

Bone regeneration above implant apex after 3 years follow up xrays for an OSFE (Osteotome Sinus Floor Elevation) using only autogenous bone core issued from trephine drilling at the implant site as grafting material, for two 6mm length implants splinted to one NC 12mm length.
