Balancing risk with possibility, this full-arch implant rehabilitation became a continuous dialogue between biology, technology, and time.

In an irradiated patient, success depended on a precise understanding of altered tissue physiology, meticulous biologic risk management, and a treatment strategy aimed at preserving vascularized bone and soft tissues.

To reduce the risk of osteoradionecrosis, no grafting procedures were performed at the time of surgery, favoring a minimally invasive approach that avoided additional surgical procedure to compromised tissues.
The treatment integrated the Straumann® Pro Arch concept with BLC Roxolid® SLActive® implants, planned through a fully digital Smilecloud × coDiagnostiX® workflow that ensured prosthetically driven implant positioning and guided, flapless surgery. High primary stability allowed for immediate loading under controlled occlusal conditions. In parallel, a pharmacologic protocol with pentoxifylline and tocopherol was implemented to enhance microvascular perfusion, reduce radiation-induced fibrosis, and support tissue healing in irradiated bone.

This case demonstrates that predictable, immediate full-arch rehabilitation is achievable even in medically compromised patients when surgical trauma is minimized, biologic risk factors are actively managed, and digital precision guides clinical ex*****on, because progress happens when technology serves biology, and time is given the chance to heal❤️‍🩹

Biology sets the boundaries, biomechanics defines the solution.

Bone volume and height is not a variable we can negotiate, but a reality we must respect. In cases of advanced mandibular atrophy, implant dentistry evolves from a question of how many implants can be placed to how forces can be safely managed.

In such scenarios , implant-supported overdenture therapy represents a predictable and effective treatment option, providing significant improvements in function, comfort, stability, and overall quality of life.

Digital treatment planning plays a fundamental role in this process. Virtual planning with CoDiagnostiX allows prosthetically driven implant positioning, precise assessment of restorative space, and early evaluation of biomechanical constraints, enabling a controlled workflow with fewer surgical and prosthetic surprises.

Within this context, two short implants (6 mm) were placed within the available anatomy and splinted with a Hader bar to enhance rigidity, load distribution, and prosthetic control. Straumann standard tissue-level implants with a 2.8 mm collar were deliberately selected over a 1.8 mm collar to improve the transmucosal profile, gain vertical height in a compromised soft-tissue environment, and reduce crestal stress.

Splinting these implants with a Hader bar converted individual implants into a unified biomechanical system, minimizing bending moments and peak stress concentrations that would be expected with unsplinted attachments.The bar pick-up phase remains one of the most delicate clinical steps, demanding meticulous blocking to prevent acrylic locking, a technically precise step.

The ultimate objective remains unchanged: to restore function and esthetics while respecting biological limits and protecting implant longevity.

This patient presented with failing dentition and reduced vertical dimension, seeking full-arch rehabilitation of both the maxillary and mandibular arches. After careful planning, implants were placed with immediate loading.
Three weeks later, the patient returned with pain and multiple implant failures , three in the maxilla and three in the mandible. Despite precise surgical and prosthetic ex*****on, post-operative instructions, including adherence to a soft diet, had not been followed.
The treatment plan was revised. In the maxilla, three new Straumann BLC implants were placed. In the mandible, the single surviving implant was now shallow relative to the resorbed bone and was therefore submerged. Four new Straumann TLC implants were placed to restore optimal positioning and support. This time, all implants were left to heal submerged, ensuring complete osseointegration before loading.
Final prosthetic rehabilitation included a maxillary titanium framework with monolithic zirconia restoration and a mandibular monolithic zirconia restoration. The maxillary impression was taken using EXACT scanbodies on SRAs, while the mandibular impression utilized metal conventional scanbodies. The maxillary workflow was model-free, whereas the mandibular workflow required a printed model and a verification jig.

A beautiful ex*****on , but this time, with a lesson: even the best plan relies on the right patient selection and clear communication.

Thoughtful preservation is as much a part of treatment as restoration itself.

This patient presented with advanced periodontal disease, facing the loss of nearly all teeth. The maxillary dentition was beyond restoration and was rehabilitated with six implants and an FP3 prosthesis, chosen to manage anticipated bone and tissue loss and to harmonize with a high smile line.
Yet the mandibular teeth told a different story, one not of replacement, but of endurance. Through surgical periodontal therapy and the gentle persuasion of orthodontics, these teeth were given a second chance .

This contrast highlights an essential principle in dentistry: treatment is not solely the act of rebuilding what has been lost, but also the careful preservation of what can be sustained. Every clinical decision requires balance between restoration and conservation, recognizing that each tooth represents not only function, but also continuity, identity, and the patient’s unique story.

This past weekend, we came together to celebrate a milestone, the 25th anniversary of the Center for Implant Dentistry. A place that’s become a second home to ITI Scholars from 21 countries.

I had the privilege of standing alongside Legends who have shaped our field, not merely with knowledge, but with vision, integrity, and generosity , , .robertlevine , . I was honored to share insights on how our field has evolved over just a quarter of a century.

Dr. Martin, Dr. Gonzaga , Dr. Ruskin,and Dr. Morton, your wisdom continues to echo in classrooms, operating rooms, lecture halls, and in the quiet moments of reflection.
Thank you for believing in us, for guiding us, and for reminding us that true influence is not loud, it is lasting. Thank you for lighting the path so many of us now walk.

Not every solution requires starting over , sometimes, the best restoration is built on what remains.

This situation was treated by implant-retained removable partial denture that combines the benefits of dental implants and removable partial dentures, offering improved stability, retention, and chewing efficiency compared to traditional RPDs. The patient did not wish to undergo extractions or pursue high-cost full-arch treatments. This treatment met her clinical needs, personal preferences, and financial considerations without compromise.

In today’s trend of defaulting to full-arch implant-supported fixed prostheses, we must remember: No treatment is superior by default but only by how well it aligns with the patient’s biology, priorities, and goals.

Whether fixed or removable, the true success of any rehabilitation lies in individualized planning, not standardized protocols.
