Expecting your baby soon while wearing braces? Here’s what patients and dentists should prepare. 🤰🦷
As your due date approaches, planning ahead helps keep your mouth healthy, your appliance comfortable, and your treatment appropriately monitored.
1. Does pregnancy mean braces must come off?
No—not simply because you are pregnant. Continuing active treatment or arranging a temporary pause depends on gum health, treatment stage, pregnancy circumstances, and your ability to attend appointments.
Pregnancy hormones can increase the gums’ response to bacterial plaque, making swelling and bleeding more likely. Cleaning around braces and monitoring gum health remain important. [1]
2. What is the best choice before delivery?
• Healthy gums and reliable follow-up: appropriately monitored treatment may continue.
• An expected interruption around childbirth: your orthodontist may consider a temporary passive holding phase.
• Gum disease, decay, pain, or appliance problems: address these and reassess the treatment plan.
A holding phase means professionally adjusting the appliance to reduce or suspend planned active movement while helping maintain progress for a defined period. It is an individualized clinical option—not a requirement for every pregnant patient.
Missing an adjustment does not automatically stop tooth movement. Wires, power chains, springs, or elastics may continue applying force.
3. What do the metal ties in these photos do?
Stainless-steel ligatures can secure the archwire within the brackets or link selected teeth, depending on their purpose.
However, metal ties do not automatically make an appliance passive. A deflected archwire or an actively tightened tie may still move teeth.
There is no single wire size or ligation pattern that is best for every patient approaching delivery. The whole appliance and its remaining forces must be assessed. Metal ties cannot replace follow-up visits.
4. What should dentists arrange before delivery?
Review gestational age, the expected delivery date, medical conditions, medications, and pregnancy complications. Coordinate with the obstetric team when indicated, especially for higher-risk pregnancies.
Keep appointments comfortable. In late pregnancy, a semi-reclined position, slight left tilt, or a small pillow under the right hip can help reduce dizziness associated with lying flat. [2]
For orthodontic care:
• Assess gum health, decay, hygiene, and irritation.
• Check brackets, bands, and wire ends; address loose or sharp components.
• If pausing active movement, evaluate all active mechanics. Changing elastic ties to steel ties alone is insufficient.
• Give clear instructions about continuing, stopping, or changing interarch elastics.
• Set the next review and provide an emergency contact.
A temporary pause may extend treatment time and still requires monitoring.
5. What should patients do at home?
Brush twice daily with fluoride toothpaste and clean between teeth daily. After vomiting, rinse with water and avoid brushing immediately. Reduce frequent sugary snacks and drinks. [3]
Interdental brushes and floss threaders can help clean around braces. Avoid hard or sticky foods that could damage the appliance. [4]
Pack a toothbrush, toothpaste, and orthodontic wax for the hospital. Wax can temporarily reduce rubbing, but broken or loose parts need professional attention. Do not tighten, cut, or adjust the appliance yourself.
6. Must braces or elastics be removed for childbirth?
Fixed braces generally do not need to be removed simply for delivery. Fixed orthodontic appliances also usually remain in place during anesthesia, but the anesthesia team must know about them.
Tell the maternity and anesthesia teams about braces, loose components, removable appliances, and elastics connecting the upper and lower teeth. Follow their instructions about removing removable devices or interarch elastics. [5,6]
7. Can necessary dental treatment be performed before birth?
Yes. Cleaning, fillings, and other necessary dental treatment can be provided during pregnancy. Clinically indicated dental X-rays and appropriate local anesthesia can also be used. Pain or infection should not be left untreated solely because the patient is pregnant. [1]
Do not self-medicate for dental pain. Avoid ibuprofen, naproxen, and diclofenac from 20 weeks of pregnancy unless specifically advised by a clinician. Prescribed low-dose aspirin for an obstetric indication is a separate exception—do not stop it yourself. [7]
8. Should the first visit after childbirth be at one month or two months?
There is no universal waiting period. An earlier visit—even before one month—can be appropriate once the mother is medically stable and comfortable travelling and sitting for treatment.
• Routine orthodontic review: If recovery is going well and the appliance has been professionally stabilized, an appointment around 4–6 weeks after delivery can be practical, depending on the orthodontist’s assessment. This is a scheduling option, not a required safety waiting period.
• Difficult recovery or complications: Significant pain after a C-section, substantial blood loss, or high blood pressure may require coordination with the obstetric team to adapt the timing and treatment.
• Dental pain, swelling, pus, or a broken appliance causing injury: Contact the clinic promptly. Do not wait for the one- or two-month mark. Difficulty breathing or swallowing requires emergency care.
Breastfeeding itself does not require postponing dental care. Inform your dentist so any necessary medications can be checked for compatibility. [8]
For dentists: Consider the total time since the last orthodontic appointment—not just the time since delivery. Assess gum health, hygiene, the appliance, and the bite before restarting active movement.
Your care can adapt to this new chapter of your life. Planning together helps you prepare for your baby with clear guidance and continued support. 💙
