🔺️درمان ارتودنسی همراه با جراحی فک در کیس کلاس Long Face III همراه با اپن بایت و انحراف کنت در فک بالا.
نکته حائز اهمیت در این کیس تبدیل Dual Occlusal Plane به Single Occlusal plane و همزمان اصلاح نسبی انحراف کنت فک بالا بدون کشیدن دندانهای آسیاب کوچک در دوره درمان ارتودنسی قبل از جراحی فک میباشد که سبب اجتناب از انجام جراحی Segmental 3 Piece در فک بالا و در نتیجه کاهش ریسک و عواقب ناشی از آن میباشد.‌

🔺️Orthodontic treatment combined with jaw surgery in a Long Face Class III case with open bite and upper jaw canting.
An important point in this case was converting the dual occlusal plane into a single occlusal plane while simultaneously achieving partial correction of the maxillary cant—without extracting premolars during the pre-surgical orthodontic phase. This approach helped avoid the need for 3-piece segmental surgery in the upper jaw, which reduced the related risks and complications.

این مورد، نمونه‌ای از انحراف جانبی فک پایین است که به‌علت عدم تقارن در قوس دندانی هر دو فک ایجاد شده بود.
در این بیمار، اصلاح عدم تقارن قوس دندانی با استفاده از انکوریج اسکلتی در ناحیهٔ کرست زیرگونه‌ای (Infra-zygomatic crest) و باکال شلف باعث شد فک پایین به وضعیت فیزیولوژیک صحیح خود بازگردد.
این امر منجر به اصلاح انحراف فک پایین و بهبود موقعیت لب‌ها شد؛ بدون نیاز به هیچ‌گونه مداخلهٔ جراحی.

مدت درمان: ۱۹ ماه

This case demonstrates a mandibular lateral deviation caused by arch asymmetry.
In this patient, correcting the arch deviation by using skeletal anchorage in the infra-zygomatic crest and buccal shelf regions repositioned the mandible to its proper physiologic position. This resulted in correction of the mandibular deviation and improvement of lip posture without any surgical intervention.
Treatment duration: 19 months.

ناهنجاری کلاس III بسیار شدید با الگوی Long face, انحراف فک پایین، کراس بایت خلفی دوطرفه و فقدان دندان شماره ۲ سمت راست در فک پایین در مدت ۲۰ ماه با استفاده از تکنیک MEAW درمان شد. در ناحیه سمت راست فک پایین به علت فقدان دندان شماره ۲، دندان نیش جایگزین دندان ۲ و دندان شماره ۴ جایگزین دندان نیش شدند.
📌 نکته کلیدی در بیومکانیک درمانی تکنیک میا، ایجاد چرخش در زاویه پلن اکلوزال میباشد. در این کیس تغییرات در زاویه پلن اکلوزال و به دنبال آن چرخش مندیبل در جهت اصلاح ناهنجاری و همچنین تغییرات بافت نرم لبها در نمای کلیشه رادیوگرافی لترال (اسلاید ۴) کاملا مشخص است.

Severe Class III Malocclusion with long face facial pattern, mandibular deviation, bilateral posterior crossbite, and missing lower right lateral incisor treated within 20 months using MEAW Technique. Lower right canine substituted for missing lateral as well as lower right 1st premolar for lower right canine.
📌The key point of biomechanics in MEAW Technique is the rotation of occlusal plane. In this case, the rotation of occlusal plane, which caused rotation of mandible in a direction to correct the Malocclusion and changes of lips posture, can be seen on lateral radiograph(slide no 4).

🔺️درمان ارتودنسی بدون جراحی فک در کیسی با مشکل کلاس II اسکلتال، لبخند لثه ای، جلوزدگی شدید دندانی دو فک و اپن بایت قدامی در مدت ۱۹ ماه انجام شد. در این کیس با استفاده از TADs system ایمپکشن ماگزیلا جهت اصلاح لبخند لثه ای و دیستالیزیشن دنتیشن هر دو فک جهت اصلاح جلوزدگی دندانی انجام شد. به دنبال ایمپکشن ماگزیلا، چرخش قدامی مندیبل به سمت قدام رخ داد که تغییرات قابل توجهی در چهره بیمار ایجاد کرد. لازم به ذکر هست جهت کسب نتیجه بهتر در نمای نیمرخ و جبران کمبود استخوان در ناحیه چانه، تنها جراحی چانه که پیش بینی آن توسط هوش مصنوعی طراحی شده و در اسلاید نمای نیمرخ گنجانده شده برای بیمار برنامه ریزی شده است.
🔺️Non surgical orthodontic treatment done for a skeletal class II case with gummy smile, bimaxillary dentoalveolar protrusion, and anterior openbite within 19 months. In this case, the TADs system is used for maxillary impaction to treat gummy smile and also for distalization of upper and lower dentition to treat bimaxillary protrusion. Note the anterior rotation of the mandible, which occurred followed by maxillary impaction and enhanced facial profile significantly. In order to achieve maximum result in facial profile and improve bone deficiency in the chin area, the chin Genioplasty was offered to the patient, which the prediction by AI software is added to the profile slide.
