recurrent juvenile parotitis

التهاب الغدة النكفية المتكرر / تحدث هذه الحالة المرضية عند الأطفال ويعانون من انتفاخات متعددة في احد الغدتين النكفيتين او كليهما مع ارتفاع درجة الحرارة احيانا .الاسباب غير معروفة . تزول هذه الحالة تلقائيا بعد البلوغ ولاتحتاج الى تداخل جراحي .

8 years old male presented with chronic recurrent unilateral parotid swelling , not related to meal time , painful and sometimes with fever. Swelling accomplished by one day and subside weeks.

Ultrasound findings : multiple tiny hypoechoic nodules ( punctate sialactasis ).

Diagnosis : " recurrent juvenile parotitis "

⁨ شاب تعرض الى حادث سير شديد ادى الى كسور متعددة في عظام الوجه جميعا شملت قاعدة الجمجمة الامامية ونضح السائل الشوكي الدماغي في اول ايام الاصابة. تم اجراء عملية جراحية فوق الكبرى في صالة عمليات الطابق الارضي في مستشفى ازادي التعليمي في كركوك شملت اصلاح وتثبيت عظام الوجه بواسطة صفائح التيتانيوم المعدنية والبراغي.

22 years old male presented with history of severe road traffic accident leading to multiple facial bone fractures involving ( Lefort 2 fracture involving bilateral infraorbital rims, nasal bone and naso - orbito ethmoid fracture, split maxilla , comminuted mandiblular symphysis fracture ).
At early time patient presented with CSF ( cerebrospinal fluid rhinorrhea ) but stopped after few days.

Open reduction and internal fixation done under general anesthesia⁩

⁨ ⁨ شاب بعمر العشرينات تعرض الى حادث سير ( دراجة نارية ) مما ادى الى كسور متعددة متحركة في الفك الاسفل .

تم تثبيت الكسور بعملية جراحية فوق الكبرى تحت التخدير العام بواسطة صفائح وبراغي التيتانيوم المعدنية في مستشفى السلام الاهلي في كركوك.

How can you deal with an oral ulcer ??

1️⃣ Duration of the ulcer is important : acute oral ulceration ( with pain ) that has less than 10 days duration is usually benign :

🌷immune origin called :
“ aphthus ulceration “.

🌷 traumatic history as bite or sharp tooth or food or due to denture trauma called :
“ traumatic ulceration “.

🌷 ulcer due to burn from chemicals as CMCP called :
“ chemical ulceration “

The shape of these ulcers are mostly the same. But differs by mechanism and initiating factor and therefore the names differ from each others according to the causative factors.

🌷 burn from heat food lead to sloughing.

If the ulcer is chronic ( month/ months duration), it is either benign ( Tuberculus ulcer, non - specific ulcer, TUGSE / traumatic ulcerative granuloma with stromal eosinophilia) or major aphthus ulceration that may lasts up to 4 weeks in order to heals completely.

Or malignant ( usually squamous cell carcinoma) or other rare oral cancers.

2️⃣ shape of the ulcer : whitish yellowish base ( center) with flat margin surrounded by red halo is a presentation of acute aphthus or traumatic ulceration.

Rolled elevated margin is a sign of malignant ulcer or sometime chronic traumatic ulceration.

3️⃣ palpation of the ulcer : hard indurated ulcer is usually malignant or somtime chronic traumatic ulcer may be of firm consistency.

Ulcer of softer texture is usually benign ( acute or chronic).

4️⃣ symptoms : painful ulcers are usually benign acute ulcer or TUGSE or non - specific ulcer that may presents with pain but malignant ulcer should not be excluded.

Painless ulcer may go with malignant ulcer ( scc ) or chronic traumatic ulceration ( that may be seen as granuloma )

🌕 كيس درمويد Dermoid cyst : هو كيس ولادي congenital ( يبدأ بالظهور منذ الولادة ) ويكبر تدريجيا بالحجم بمرور الوقت.

🌕 سبب ظهور هذ الكيس في هذا المكان بالتحديد هو انحسار الانسجة الرخوة في منطقة التقاء عظم الوجنة مع عظم الجبهةfrontozygomatic suture .

🌕يكون مكان هذا الكيس عميقا ( فوق سمحاق العظم و تحت العضلة الغالقة للعين supraperiosteally and submuscularle)

🌑 تمت عملية ازالة الكيس تحت التخدير الموضعي في عيادتنا الخاصة بواسطة جرح اسفل الحاجب infrabrow incision علما ان عمر الطفل ثلاثة سنوات.
