🌍 All-on-6 with Pterygoid Implants for an NRI Patient 🇺🇸 with BIL-RAM™️ Protocol.

🌟A middle-aged woman residing in the USA visited us for comprehensive dental implant rehabilitation. With her return journey scheduled within a Month, the entire treatment had to be completed within her limited stay in India. ✈️
🦷 The procedure involved immediate extractions, immediate implant placement, and immediate loading, enabling the patient to receive fixed teeth without waiting for several months.
🔩 The case was executed according to My BIL-RAM™ Protocol — Bicortical Immediate Loading with Remote Anchorage on Multi-unit abutments. Six implants were strategically positioned, including bilateral pterygoid implants to provide strong posterior remote anchorage and improve the anteroposterior spread.
All the implants used were Neodent, except for the pterygoid implants.

🔥 PATZI Protocol—once again!
🦷 This time, the maxillary reconstruction was performed using 3 implants engaging the nasal floor/nasalis region, 2 zygomatic implants, and 2 pterygoid implants.Fixed teeth in just 8 days.
⚡ The patient presented with aggressive periodontitis and was managed with immediate extractions, immediate implant placement, and immediate loading.
🔩 All implants used were crestally polished implants, except for 2 placed in the 36 and 46 regions.

🌟QUAD ZYGOMATIC IMPLANTS | 6-MONTH FOLLOW-UP ✅

⭐️This challenging Quad Zygoma rehabilitation was performed by me in Mumbai as a consultant in January 2026 and i thank Dr Piyush Upadhayay & Dr Shraddha Ekkaldevi for the opportunity & the lower jaw and all the prosthetic steps were done by Dr piyush & Dr shraddha beautifully.
⭐️The patient had a severely resorbed maxilla with a knife-edge ridge and D4 bone quality. She had previously been advised against conventional implant treatment elsewhere because of the poor available bone and was given an ill-Fitting denture.

🌟Post-operative CBCT was performed at an independent scan centre owned by the Oral Medicine and Maxillofacial Radiology specialist who subsequently evaluated the scan. He prepared an elaborate radiology report extending to nearly 20 pages, assessing the implant trajectories, zygomatic engagement, sinus relationship, and surrounding anatomical structures (slices of which is posted here)
🌟i actually fed this PDF to chat gpt and it gave me a surgeon score of 9/10 overall (by analysing what the radiologist wrote as a summary and also looking at all the slices posted by those Docs in the report itself...
🦷A quad zygomatic implant approach was planned to obtain remote anchorage and establish a stable full-arch foundation.
🔹 In the first quadrant, zygomatic implants were positioned in the canine and first-molar regions.
🔹 In the second quadrant, zygomatic implants were positioned in the first-premolar and molar regions.
🔹 The strategic distribution helped achieve favourable anterior–posterior spread and cross-arch stability for rehabilitation.

🌟Now at the 6-month follow-up, the patient is doing well clinically, and all four zygomatic implants remain stable and healthy and the patient is extremely happy to get rid of her denture.she is chewing all the food she likes to.

🦷 ALL-ON-X IMMEDIATE LOADING | HYDERABAD SOFTWARE ENGINEER

👨‍💻 This full-arch immediate-loading rehabilitation was done for a software engineer from Hyderabad, referred by my best friend, roommate, and PSIMS Lion, Dr. Mukkapati Koteswara Rao, M.S. (General Surgery).

🏗️ The maxilla had deficient bone, so the implant plan included nasal-floor engagement and bilateral pterygoid anchorage & a Nasopalatine implant to obtain a stable remote-anchorage foundation for immediate loading.
✨ The final restoration was a screw-retained zirconia monolithic prosthesis over a custom I-bar framework on multi-unit abutments.

🎯 What made this case particularly special was the level of customization involved. The patient was very particular about the tooth form, smile, phonetics, and overall appearance. Multiple trial corrections were carried out until he was completely satisfied.

🖥️ He literally sat beside me with the 3D-printed screw-retained trial prosthesis in his mouth, dictating the changes he wanted, while the design was modified in real time.

🌟 I-bar framework extraction from the wax-up using Blender for Dental (B4D)
🌟 CAM designing in MillBox

🌟 Final glaze and gingival ceramic layering completed by our in-house technician

The implants used were AB Dental implants, with Osstem implants in the lower anterior region. The pterygoid implants were Quickdent.

👨‍⚕️ From implant placement and biomechanics to digital smile design, CAD/CAM design, milling, sintering, and final finishing, every critical step was closely controlled without compromise.

🌟PATZI Protocol in a maxilla with highly pneumatized sinuses.
🌟I even forgot to say him that his upper jaw has less bone and so we are going for zygomatic implants...I just explained the process and they proceeded.
🌟A 64-year-old male patient reported with multiple root stumps and In the same surgical appointment, all root stumps were removed and immediate implants were placed using the PATZI Protocol — combining:

When Failure Happens Twice… We Don’t Give Up.
This patient came to us from Nalgonda District, Telangana after facing two major implant failures done elsewhere.
▪️ First: Conventional implants – failed after 3 years.
▪️ Second: Full mouth BASAL implants – failed within just 6 months.
After removal of all implants, he was advised a removable prosthesis… something he simply couldn’t accept.
As a last resort, he reached us.
Given the severe maxillary compromise, I suggested Zygomatic implant rehabilitation — the ideal solution in such cases of advanced atrophy. However, due to fear of zygoma implants, he declined.
With utmost surgical planning and careful utilization of the available maxillary bone and pterygoid engagement, we proceeded with an All-on-6 rehabilitation achieving solid bicortical engagement for enhanced primary stability.
✅ Today marks his 4-month follow-up, and I’m happy to share that all implants are stable and doing well.
Though I still believe zygomatic implants would have been biomechanically ideal in this case, respecting patient comfort and psychology is equally important in treatment planning.
