The discovery of insulin in 1921 is one of the major breakthroughs in medicine of the 20th century. It gave a new lease of life to millions of people with diabetes all over the world. Insulin is the sole pharmacological therapy which is effective in type1 diabetes and is essential for survival of the patients. Most patients of type 2 diabetes also require insulin sooner or later in order to achieve acceptable control of blood sugar.
But, in spite of so much benefit, a number of barriers to insulin therapy exist:-
1. Cost
2. Fear of hypoglycemia
3. Pain and Needle phobia
4. Inconvenience
5. Non realization of the seriousness of the consequences of diabetes
6. Fear of weight gain
7. Need to monitor blood glucose more frequently
8. False belief that insulin is only for serious cases
There also exists a psychological resistance both from patients and treating doctors.
1. Patients
• “Once on insulin I will get addicted to it.”
• “Insulin is the last tool to use”
• “Insulin means frequent hypos”
• “No doctor…. Give me 1 more month, I will show you”
• “Once on insulin… always on insulin”
2. Doctors
• “Insulin has many side effects… even bone thinning”
• “If you do not show good control, next time I will put you on insulin”…. A punishment to the patient and guilt.
