Saving a Smile: Re-Evaluating the Plan When Time is of the Essence ⏳
​This 9-year-old patient arrived with severe pain in their lower left first molar. The initial assessment and radiographs showed massive decay. A tooth that critical at this young an age must be saved.
​Initial Plan: MTA Pulpotomy. The hope was to perform a simple pulpotomy to preserve the remaining pulp and allow root development.
​Real-World Challenge: Upon entering the pulp chamber, the tissue was clearly necrotic. A pulpotomy was no longer viable. The success of the case depended on immediate adaptation.
​Revised Plan: Full Root Canal Treatment (RCT). I navigated the canals, but the anatomy was wide and still developing. I located some vital pulp tissue near the apex, which is a positive sign. The final fill was completed 1mm short of the apex.
​Success: The treatment alleviated the severe pain, and the tooth is now sealed with a multi-layered composite build-up (check that clean final look!). This tooth now has a fighting chance to last a lifetime.
