Case Presentation: Endodontic Management of Tooth #46
Endodontic treatment was performed on tooth #46 (mandibular right first molar) presenting with complex root canal anatomy comprising five canals.
Following access cavity refinement, five canals were identified and negotiated:
Mesiobuccal (MB)
Mesiolingual (ML)
Middle Mesial (MM)
Distobuccal (DB)
Distolingual (DL)
Working length was determined using an electronic apex locator and confirmed radiographically. Cleaning and shaping were performed using the NT File System, preserving the original canal morphology.
Copious irrigation was carried out using Cerkamed sodium hypochlorite. All canals were subsequently three-dimensionally obturated using the Woodpecker obturation pen system with a bioceramic sealer (Raident), achieving a dense and hermetic apical and coronal seal.
This case highlights:
Clinical relevance of identifying middle mesial canals in mandibular molars
Importance of proper access cavity design and canal scouting
Effective use of contemporary rotary instrumentation, bioceramic sealers, and warm obturation techniques in complex cases
