The placement of a “cap” or crown involves careful reshaping the tooth to create the right space and form for the crown to sit securely.

This reduction is not random and just removing and cutting in a circular pattern is NOT scientific. It poses short term effects like sensitivity or long term ones like fracture or loss of retention.

It follows precise mechanical as well as biological principles to ensure:
• Proper fit
• Strong support
• Healthy gums
• Long-term durability

𝐈𝐌𝐌𝐄𝐃𝐈𝐀𝐓𝐄 𝐒𝐔𝐑𝐆𝐈𝐂𝐀𝐋 𝐎𝐁𝐓𝐔𝐑𝐀𝐓𝐎𝐑

When part of the upper jaw is removed during surgery eg. due to cancer, it creates an opening between the mouth and the nose.
An immediate surgical obturator is a Prosthesis that is prepared before surgery and placed immediately after the operation.

Role:
• Separates oral and nasal cavities immediately
• Enables early speech and swallowing
• Supports surgical dressings and hemostasis
• Maintains facial contour
• Better psychological and functional recovery

In Partial Maxillectomies, where teeth is retained after surgery, the obturator can take easy support from adjacent teeth with the help of clasps. But cases like total maxillectomies or edentulous ridges pose a challenge.

Various modifications can be done based on the case and suitability of the surgeon. Holes punched near the borders of the prosthesis to enable passing of suture. Wire loops for pack support and even be removed by mosquito forceps if not required. Trimming of lateral areas for better adaptation, etc.

The surgical obturator is delivered immediately post-resection, restoring separation between oral and nasal cavities, supporting dressings, and enabling early function and support.

Nature follows patterns.

The 𝗙𝗶𝗯𝗼𝗻𝗮𝗰𝗰𝗶 𝘀𝗽𝗶𝗿𝗮𝗹 and 𝗚𝗼𝗹𝗱𝗲𝗻 𝗣𝗿𝗼𝗽𝗼𝗿𝘁𝗶𝗼𝗻 (≈𝟭.𝟲𝟭𝟴) appear in flowers, shells, art, architecture and subtly in an attractive smile.

This concept of proportions helps us to guide tooth width, length, and symmetry while planning aesthetic restorations and veneers.

They are not rigid rules, but biological references create smiles that look natural, balanced, and harmonious. Not artificial.

𝐈𝐌𝐏𝐋𝐀𝐍𝐓𝐎𝐋𝐎𝐆𝐘- 𝐓𝐫𝐚𝐧𝐬𝐟𝐨𝐫𝐦𝐢𝐧𝐠 𝐂𝐨𝐧𝐯𝐞𝐧𝐭𝐢𝐨𝐧𝐚𝐥 𝐃𝐞𝐧𝐭𝐮𝐫𝐞𝐬 𝐭𝐨 𝐂𝐨𝐧𝐟𝐢𝐝𝐞𝐧𝐭 𝐒𝐦𝐢𝐥𝐞𝐬

Conventional Lower (mandibular) dentures often have poor retention and stability leading to patient discomfort and dissatisfaction. This is specially true for severely resorbed ridges. Some patients who don't follow up, even completely avoid using lower dentures.

A simple use of implants can be an effective treatment option.

Just two Implants placed in the canine region can maximize support and minimize rotational movement and be better than its conventional counterpart.

Here, the same lower denture has been used and modified to place attachments (Locator type). Dual Scan was also performed which merges scans of the patient and denture radiograph for precise, prosthetically driven implant placement.

Patient regained comfort, function and confidence.

This is one of my paper presentation which I delivered at a national conference in my PG days. It talks about a beautiful , often lesser known niche in dentistry in very simple words.

How the alignment of the jaw and bite affects posture, which in turn influences the entire kinematic chain and the coordinated movement of the body.

Sports Dentists are specialized professionals who form a part of the medical support team for Athletes and high performers where correcting occlusion via occlusal therapy can enhance postural stability, balance, and neuromuscular coordination, lowering the risk of injury and boosting performance.

𝐈𝐌𝐏𝐋𝐀𝐍𝐓𝐎𝐋𝐎𝐆𝐘

Implants are one of the fastest growing and most extensively researched fields in modern dentistry. As an introductory post, I will not delve into the vast details of materials or surgical technique but instead I want to draw your attention to a beautiful integration.

It is the remarkable union between Bone, a living, dynamic tissue, with a foreign material—the implant. How extraordinary that bone not only accepts but biologically bonds with this implant through the process of osseointegration, ultimately recognizing it as part of the body.

To truly appreciate this phenomenon, we need to understand its three essential elements:

𝗧𝗵𝗲 𝗕𝗼𝗻𝗲 – constantly remodeling, responsive, and alive.
𝗧𝗵𝗲 𝗜𝗺𝗽𝗹𝗮𝗻𝘁 – precisely engineered from biocompatible materials
𝗧𝗵𝗲 𝗕𝗼𝗻𝗲–𝗜𝗺𝗽𝗹𝗮𝗻𝘁 𝗜𝗻𝘁𝗲𝗿𝗳𝗮𝗰𝗲 – the microscopic connection where biology meets biomaterial.

Such a perfect harmony between science and nature is what makes implantology truly inspiring.

Even in the absence of any prosthesis, the patient was unable to fully close the left eyelid (lagophthalmos), despite voluntary effort. The upper eyelid appeared retracted and stiff, likely as a result of long-term use of an oversized ocular prosthesis initiated soon after enucleation. This is an uncommon complication, distinct from the more frequently observed issues such as ptosis, ectropion, or socket contraction.

A surgical correction before prosthesis could provide a better outcome but patient was unwilling to undergo any invasive procedures.
