The placement of a “cap” or crown involves careful reshaping the tooth to create the right space and form for the crown to sit securely.

This reduction is not random and just removing and cutting in a circular pattern is NOT scientific. It poses short term effects like sensitivity or long term ones like fracture or loss of retention.

It follows precise mechanical as well as biological principles to ensure:
• Proper fit
• Strong support
• Healthy gums
• Long-term durability

When part of the upper jaw is removed during surgery eg. due to cancer, it creates an opening between the mouth and the nose.
An immediate surgical obturator is a Prosthesis that is prepared before surgery and placed immediately after the operation.

Role:
• Separates oral and nasal cavities immediately
• Enables early speech and swallowing
• Supports surgical dressings and hemostasis
• Maintains facial contour
• Better psychological and functional recovery

In Partial Maxillectomies, where teeth is retained after surgery, the obturator can take easy support from adjacent teeth with the help of clasps. But cases like total maxillectomies or edentulous ridges pose a challenge.

Various modifications can be done based on the case and suitability of the surgeon. Holes punched near the borders of the prosthesis to enable passing of suture. Wire loops for pack support and even be removed by mosquito forceps if not required. Trimming of lateral areas for better adaptation, etc.

The surgical obturator is delivered immediately post-resection, restoring separation between oral and nasal cavities, supporting dressings, and enabling early function and support.
