🦷 When a Small Lesion Becomes a Major Structural Challenge: Cemento-Ossifying Fibroma (COF)
What may begin as a subtle change within the periodontal ligament can gradually develop into a well-defined, expansile lesion capable of significantly altering the surrounding anatomy.
🔬 Understanding the Pathology
Cemento-Ossifying Fibroma is a benign fibro-osseous neoplasm in which normal bone is progressively replaced by fibrous tissue along with varying amounts of calcified material. Its growth is generally slow but can be persistent and expansive.
As the lesion increases in size, adjacent teeth and anatomical structures may become displaced. In advanced cases, remarkable tooth movement can occur, while root resorption may not always be a prominent feature.
⚕️ Important Differential Diagnoses
1️⃣ Cementoblastoma – Typically remains intimately associated with the tooth root and may be accompanied by pain or tenderness.
2️⃣ Fibrous Dysplasia – Usually demonstrates a less-defined transition with a characteristic “ground-glass” radiographic pattern, unlike the sharply demarcated appearance often seen in COF.
3️⃣ Periapical Cemento-Osseous Dysplasia – Generally presents as a smaller, self-limiting lesion, commonly involving the anterior mandible.
🛠️ The Surgical Principle
Because COF is often well circumscribed and encapsulated, surgical enucleation can allow the lesion to be separated from the surrounding healthy bone. Depending on the resulting defect, reconstruction with bone grafting and a resorbable membrane may be considered to support restoration of mandibular form and function.
