Transforming Airway & Smile Together – A 10-Year-Old’s Progress

This 10-year-old boy visited our clinic after undergoing tongue-tie release and adenoidectomy, performed by his ENT surgeon parents.

While the surgeries addressed the soft-tissue restrictions, his jaws presented maxillary constriction and a deep, narrow palate, which develop over years of mouth breathing.

Our Treatment Approach

Before any appliance therapy, we first began with:
Oro-Myofunctional Therapy (OMT) – to correct tongue posture, breathing pattern, and swallowing.

Once the muscles were retrained, we initiated:

Modified Hyrax Rapid Maxillary Expander (RME) – to widen the maxilla and recreate healthy tongue space.

The lower incisors were retroclined initially, but we did not provide any treatment for the lower arch.
As the maxilla expanded, functional forces improved, and tongue posture normalized…
The lower arch aligned naturally!

Wider maxillary arch
Better palatal form
Increased tongue space
Improved airway function
Natural lower arch alignment

Early intervention allows us to:

Guide jaw growth naturally
improve airway and sleep
prevent future crowding and crossbites
avoid aggressive treatment later

🌟 Case of the Day: Natal Tooth Extraction in a 15-Day-Old Baby! 👶🦷

Did you know that some babies are born with teeth? These are called Natal Teeth, and while they are rare, they can sometimes cause feeding difficulties or discomfort for the baby and mother.

🔹A 15-day-old newborn came to our clinic with a mobile lower front tooth present since birth. The parents were concerned as it was affecting breastfeeding and tongue irritation.

🔹 What We Did: After careful evaluation, we found the tooth to be highly mobile with no proper root formation, posing a risk of accidental swallowing or aspiration. A safe and gentle extraction was performed under proper aseptic conditions, ensuring the baby’s comfort.

🔹 Why It’s Important
✅ Prevents choking hazards.
✅ Improves breastfeeding for the baby and mother.
✅ Prevents tongue ulcers (Riga-Fede disease).

Today, we’re sharing the case of a 3-year-old boy who underwent extraction of tooth 64 and restoration of teeth 65 and 54 under oral sedation, specifically tailored for children with severe anxiety and uncooperativeness.

The first section of the video is shown at normal speed, providing a real-time view of how oral sedation effectively calms the child.

The subsequent parts are presented at 1.5x speed, focusing on the extraction and restorative procedures, while shortening the viewing time.

When Laughing Gas is Ineffective or Not Feasible: For children with extreme anxiety or uncooperative behavior, laughing gas (nitrous oxide) may not be sufficient to achieve the desired level of calmness. In such cases, oral sedation becomes a more appropriate option, as it provides deeper relaxation and helps manage anxious or uncooperative patients effectively.

Enhanced Calm and Cooperation: Oral sedation beneficial for children as it allows them to remain calm and relaxed throughout the procedure, making it easier for the dental team to perform treatment without resistance or distress.

Safety and Monitoring: The child’s vital signs were carefully monitored during the procedure, with pre-sedation assessments and post-operative care to ensure the highest level of safety and comfort.

Efficient Multi-Procedural Care: With oral sedation, we were able to complete the extraction and restorations in one session, reducing the need for multiple visits, which can further stress anxious children and their parents.

The video is accompanied by pre-sedation, intra-sedation, and post-treatment records, documenting the child’s journey through the sedation process.

This case highlights how oral sedation offers a safe and effective solution for young children with severe anxiety or uncooperativeness, especially when alternatives like laughing gas are not suitable.

📸🦷 Crafting a DIY Tongue Tie Release Elevator for Laser Surgery 🪓

Dear colleagues

I recently encountered a situation where I needed a specialized wooden tongue tie release elevator for a laser surgery, but couldn't find one readily available. So, I decided to put on my DIY cap and create one myself! 🔨

Here's a quick rundown of how I crafted this tool:

1. **Materials**: I started with a sterile tongue depressor

2. **Design**: I carefully shaped the tongue depressor to create a smooth, rounded end with a slight curve. I used acrylic trimming bur to shape it. This design is essential to ensure patient comfort and precise surgical access.

3. **Finishing Touches**: After creating the desired shape, I made sure all edges were smooth and free from any potential sharp points. Fir finishing I used Shofu composite polishing discs.

4. **Sterilization**: Finally, I autoclaved the DIY tongue tie release elevator & it was ready for clinical use.
